Red spots on the foreskin issue?

Hello Dr.
Sun! I would like to ask: Recently, I applied "Betadine ointment" to the foreskin and glans, and after a few days, red spots began to appear on the foreskin, increasing in number and becoming concentrated.
Yesterday (10/9), the color changed from red to light yellow, and sometimes there is a slight itch.
When I press on it, it can be painful at times and sometimes not.
What could these symptoms indicate? Thank you for your attention, Dr.
Sun!

Dr. Sun Hongshun reply Urology

Hello: I apologize for the delayed response due to other commitments and extremely unstable internet connectivity in that area.
It is possible that the original skin lesion is the cause, or it could be an allergic reaction to the ointment.
I recommend consulting a dermatologist for further evaluation.
Wishing you good health.

Red spots on the foreskin can be concerning and may arise from various causes. Given your recent use of "Betadine" ointment, it's essential to consider both the potential effects of the medication and other underlying conditions that could contribute to the symptoms you're experiencing.

1. Irritation or Allergic Reaction: The application of topical ointments like Betadine can sometimes lead to skin irritation or an allergic reaction, especially if used frequently or in sensitive areas. Symptoms may include redness, itching, and discomfort. If the red spots appeared shortly after applying the ointment, this could be a likely cause.

2. Infection: The presence of red spots that change color (from red to yellow) could indicate a possible infection, such as a yeast infection or bacterial infection. Yeast infections, in particular, are common in moist areas and can cause symptoms like redness, itching, and discharge. If the yellow color is accompanied by discharge, it might suggest a bacterial infection, which could require medical evaluation and treatment.

3. Dermatitis: Conditions like contact dermatitis can occur due to irritants or allergens. This condition can present with red spots, itching, and sometimes pain. If you have been using any new products (soaps, lotions, or even laundry detergents) that come into contact with the area, they could be contributing to your symptoms.

4. Pearly Penile Papules: These are small, benign bumps that can appear on the penis and are often mistaken for other conditions. They are typically harmless and do not require treatment. However, they usually do not cause pain or significant discomfort.

5. Other Conditions: Other dermatological conditions, such as psoriasis or lichen planus, can also manifest as red spots or lesions on the genital area. These conditions may require specific treatments and should be evaluated by a healthcare professional.

Recommendations:
- Avoid Irritants: If you suspect that the ointment may be causing irritation, discontinue its use and observe if the symptoms improve.
- Maintain Hygiene: Keep the area clean and dry. Avoid using harsh soaps or products that may irritate the skin further.

- Monitor Symptoms: Keep an eye on the symptoms. If the redness persists, worsens, or if you develop additional symptoms such as pain, increased itching, or discharge, it is crucial to seek medical attention.

- Consult a Healthcare Provider: Given the symptoms you described, particularly the pain and the change in color of the spots, it would be wise to consult a healthcare provider or a dermatologist. They can perform a physical examination and may recommend tests to determine the exact cause of your symptoms.

In summary, while the red spots on the foreskin could be due to irritation from the ointment, they may also indicate an underlying infection or dermatological condition. Monitoring the symptoms and seeking professional advice will help ensure appropriate treatment and management.

Ha! You're overthinking it. Since you've had this since childhood, it's unlikely to be a "sexually transmitted infection." Please ask your dad to take you to the hospital to see a urologist!
