Right abdominal issues?
Hello doctor, I have been experiencing pain in my right abdomen for about three to four days.
The pain is located just below the rib cage, closer to the right side (if I touch from my shoulder down to below the rib cage).
What could be the issue?
Niming, 20~29 year old female. Ask Date: 2017/03/16
Dr. Wu Fangcan reply Family Medicine
According to your description, the area under the right upper rib cage is approximately the location of the gallbladder or liver.
You may consult a hepatobiliary specialist or undergo an abdominal ultrasound examination.

More Info
Right abdominal pain can be a complex issue, as it can arise from various underlying conditions. Given that you've been experiencing this pain for three to four days, it's important to consider several potential causes and when you should seek medical attention.
Possible Causes of Right Abdominal Pain
1. Gallbladder Issues: One of the most common causes of pain in the right upper quadrant (RUQ) is related to the gallbladder. Conditions such as gallstones or cholecystitis (inflammation of the gallbladder) can lead to sharp pain, especially after eating fatty meals. This pain may radiate to the right shoulder or back.
2. Liver Conditions: Since the liver is located in the RUQ, any liver-related issues, such as hepatitis or liver abscess, can cause discomfort or pain in this area. Given your history of chronic hepatitis B, it is essential to monitor any new or worsening symptoms closely.
3. Kidney Problems: Conditions such as kidney stones or a kidney infection (pyelonephritis) can also present as pain in the right side of the abdomen. This pain may be accompanied by other symptoms like urinary changes or fever.
4. Gastrointestinal Issues: Disorders such as appendicitis (though typically presenting with pain in the lower right abdomen), diverticulitis, or even peptic ulcers can manifest as abdominal pain. If the pain is associated with nausea, vomiting, or changes in bowel habits, these conditions should be considered.
5. Musculoskeletal Pain: Sometimes, pain in the abdominal area can be due to muscle strain or injury, especially if you have engaged in physical activity that may have strained the abdominal muscles.
6. Other Considerations: Other less common causes could include hernias, pancreatitis, or even referred pain from conditions affecting the lungs or heart.
When to Seek Medical Help
You should seek medical attention if:
- The pain is severe and persistent.
- You experience additional symptoms such as fever, chills, jaundice (yellowing of the skin or eyes), or changes in your bowel or urinary habits.
- There is any sign of gastrointestinal bleeding, such as black or bloody stools or vomiting blood.
- You have a history of liver disease, and the pain is accompanied by symptoms like increased fatigue, confusion, or swelling in the abdomen or legs.
Diagnostic Approach
When you visit a healthcare provider, they will likely perform a thorough history and physical examination. They may order imaging studies such as an ultrasound or CT scan to evaluate the organs in the abdominal cavity. Blood tests may also be conducted to check for signs of infection, liver function, or other abnormalities.
Conclusion
In summary, right abdominal pain can stem from various causes, ranging from benign to serious conditions. Given your symptoms and the duration of the pain, it is advisable to consult a healthcare professional for a comprehensive evaluation. Early diagnosis and treatment can significantly improve outcomes, especially in cases of acute conditions like gallbladder disease or appendicitis. Always trust your instincts about your health, and do not hesitate to seek help when something feels off.
