Diarrhea and weight issues?

I have been experiencing persistent diarrhea for the past four days.
My stools have been somewhat formed, resembling mud, but at times they are watery.
Normally, I have a bowel movement once every morning and drink 1000 to 2000 cc of water daily.
However, during these last four days, I have been experiencing frequent abdominal pain, leading to bowel movements about four times a day, with watery stools that are golden yellow in color.
After wiping with toilet paper, it feels wet.
Additionally, I have essential tremor and have been taking medication to help manage the shaking, specifically Propranolol and Clonazepam.
Could the side effects of these medications be related to my diarrhea?
Furthermore, my height is 168 cm and my weight is 45 kg, which I feel is very underweight as it has remained stagnant at 45 kg.
I maintain a balanced diet with three meals a day but have been unable to reach 50 kg.
Before the diarrhea, I had a significant amount of stool.
I am unsure if this is related to the frequency of my bowel movements.
Last July, I had a blood test during a health check-up, and my hemoglobin and triglyceride levels were both below the standard values.
The doctor advised me to eat more.
Additionally, when I squat and then stand up after a few seconds, my vision goes dark and I feel dizzy for about two seconds before returning to normal.
This seems like it could be related to anemia.
Could this be connected to my low hemoglobin levels?

Hello: If symptoms occur recently while on long-term medication, they are likely not caused by the medication.
Sometimes, changing positions can lead to temporary orthostatic hypotension, which is not necessarily related to hemoglobin levels.
If you have not been evaluated for endocrine issues, it is advisable to consult a specialist in metabolic disorders and inform them of your symptoms.

Understanding diarrhea and weight issues from a gastroenterology perspective involves examining various factors that could contribute to gastrointestinal symptoms and weight fluctuations. Based on your description, you are experiencing persistent diarrhea, abdominal pain, and significant weight concerns, which are important symptoms that warrant further investigation.

Diarrhea and Its Causes
Diarrhea can be classified into acute and chronic types. Acute diarrhea often results from infections (viral, bacterial, or parasitic), food intolerances, or medications. Chronic diarrhea, on the other hand, can be due to conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), malabsorption syndromes, or even functional gastrointestinal disorders.

In your case, the fact that you have been experiencing diarrhea for four days, with stool varying from muddy to watery, suggests an acute episode. The presence of abdominal pain that resolves after bowel movements is also indicative of a possible functional gastrointestinal disorder, such as IBS. IBS is characterized by abdominal discomfort associated with altered bowel habits, including diarrhea and constipation.

Weight Issues
Your reported weight of 45 kg at a height of 168 cm indicates a body mass index (BMI) of approximately 15.9, which is considered underweight. This can have several implications for your health, including nutritional deficiencies, weakened immune function, and potential complications related to your primary tremor condition. The inability to gain weight despite a balanced diet could be attributed to several factors, including malabsorption due to gastrointestinal issues, increased metabolic demands, or psychological factors such as anxiety or depression.

Medication Side Effects
You mentioned taking Propranolol and Clonazepam for your primary tremor. Both medications can have gastrointestinal side effects. Propranolol, a beta-blocker, is generally well-tolerated but can occasionally cause gastrointestinal upset, including diarrhea. Clonazepam, a benzodiazepine, can also lead to gastrointestinal disturbances, although it is less common. If you suspect that your medications may be contributing to your diarrhea, it is crucial to discuss this with your prescribing physician. They may consider adjusting your medication regimen or exploring alternative treatments.

Nutritional Considerations
Given your low weight and the history of diarrhea, it is essential to ensure that you are receiving adequate nutrition. Chronic diarrhea can lead to malabsorption of nutrients, which may explain your difficulty in gaining weight. It might be beneficial to consult a registered dietitian who specializes in gastrointestinal health. They can help you develop a meal plan that focuses on nutrient-dense foods, which can support weight gain and overall health.

Potential Anemia
You mentioned experiencing dizziness upon standing, which could be a sign of orthostatic hypotension or anemia. Low hemoglobin levels can lead to fatigue and dizziness, especially when changing positions. If your blood tests indicated low hemoglobin levels, it would be prudent to investigate further. Anemia can result from various causes, including nutritional deficiencies (iron, vitamin B12, folate), chronic disease, or gastrointestinal bleeding.

Conclusion
In summary, your symptoms of diarrhea, weight loss, and potential anemia warrant a comprehensive evaluation by a gastroenterologist. They may recommend further diagnostic tests, such as stool studies, blood tests, or imaging studies, to identify the underlying cause of your symptoms. Additionally, addressing your nutritional needs and considering the impact of your medications on your gastrointestinal health will be crucial in managing your condition effectively. Please ensure to follow up with your healthcare provider for a tailored approach to your health concerns.
