Persistent Red Spots: Seeking Solutions After Multiple Dermatologist Visits - Dermatology

I have consulted multiple dermatologists, but the red spots have not disappeared?

Hello, doctor.
I have multiple small red spots on my body, some of which are expanding and appear as red circles.
This condition has persisted for several years, and I have consulted multiple dermatologists.
Most of them said it was caused by flea bites and prescribed topical medications.
I have been using them for a long time, but the red spots remain unchanged and have not faded! Only one doctor mentioned it could be microvascular dilation.
I am unsure what to do next.

Niming, 20~29 year old female. Ask Date: 2017/04/02

Without actually seeing the lesion, it is impossible to determine the issue.
If it is causing significant distress, please seek medical attention as soon as possible.
A proper examination is necessary to identify the problem and determine how to address it.

Persistent red spots on the skin can be a frustrating and concerning issue, especially when they have been present for an extended period and have not responded to treatment. Based on your description, it seems that you have sought help from multiple dermatologists, yet the problem persists. This situation can lead to confusion and anxiety, particularly when the diagnosis varies from one doctor to another.

Firstly, it's essential to understand that red spots on the skin can arise from various causes, including allergic reactions, infections, inflammatory conditions, or vascular issues such as telangiectasia (microvascular dilation). The fact that some of these spots expand and resemble a red circle may suggest an inflammatory process or a reaction to an external factor, such as an insect bite, as some doctors have indicated.

Given that you have been prescribed topical treatments without significant improvement, it may be time to consider a few additional steps:
1. Comprehensive Evaluation: If you haven't already, consider seeking a comprehensive evaluation from a dermatologist who specializes in complex skin conditions. They may perform a thorough physical examination and possibly recommend additional tests, such as skin biopsies or allergy testing, to rule out underlying conditions.

2. Consider Differential Diagnoses: Conditions like eczema, psoriasis, or even chronic urticaria could present with persistent red spots. It's crucial to explore these possibilities with your dermatologist. If microvascular dilation (telangiectasia) is suspected, treatments such as laser therapy may be beneficial.

3. Lifestyle and Environmental Factors: Reflect on any lifestyle or environmental factors that may contribute to your skin condition. Allergens, irritants, or even dietary factors can play a significant role in skin health. Keeping a diary of your symptoms in relation to your activities, diet, and any products you use may help identify triggers.

4. Topical Treatments: While you mentioned using topical medications, it may be worth discussing with your dermatologist the possibility of trying different formulations or active ingredients. For instance, products containing corticosteroids can help reduce inflammation, while those with retinoids can promote skin turnover and healing.

5. Systemic Treatments: If topical treatments are ineffective, systemic medications may be necessary. Depending on the underlying cause, options could include antihistamines for allergic reactions, antibiotics for infections, or immunosuppressive agents for inflammatory conditions.

6. Follow-Up and Monitoring: Regular follow-up appointments with your dermatologist are crucial. Skin conditions can evolve, and ongoing monitoring can help adjust treatment plans as needed.

7. Patient Advocacy: If you feel that your concerns are not being adequately addressed, don't hesitate to seek a second opinion. It's your right as a patient to advocate for your health and seek the best possible care.

In conclusion, persistent red spots on the skin warrant a thorough investigation to determine the underlying cause. While it can be frustrating to navigate through various diagnoses and treatments, working closely with a knowledgeable dermatologist and considering a comprehensive approach can lead to better management of your condition. Remember, skin health is a vital aspect of overall well-being, and finding the right treatment may take time and persistence.
