The testicles have become more swollen after taking the antibiotics prescribed by the doctor!

Hello: I went to the doctor yesterday due to pain in my left testicle.
After a urine test and a physical examination, the doctor diagnosed me with epididymitis.
The doctor prescribed antibiotics and pain-relieving anti-inflammatory medication.
After taking the medication for a day, I still feel significant pain in the testicle, and it has noticeably swollen even more! Is this a misdiagnosis by the doctor, or is this normal?

Rick, 20~29 year old female. Ask Date: 2017/04/06

Dr. Du Shixi reply Urology

Hello: Orchitis typically takes a few weeks to resolve completely.
In severe cases, it can be larger than an orange.
If there is persistent swelling, redness of the skin, or other symptoms such as fever and chills, it is important to seek medical attention from a urologist.
If it is a holiday, it is safer to go to the emergency room.
Additionally, spicy foods, irritants, and alcohol should be avoided, and it is important to stay well-hydrated.

Swelling of the testicle after taking antibiotics can be concerning, especially if you are experiencing significant pain and an increase in size. Based on your description, it seems that you have been diagnosed with epididymitis, which is an inflammation of the epididymis, the tube that carries sperm from the testicle. This condition can be caused by infections, including bacterial infections, which is why your doctor prescribed antibiotics.

When you start taking antibiotics, it is important to understand that they may not provide immediate relief. Antibiotics work by targeting the bacteria causing the infection, but it can take time for the medication to reduce inflammation and alleviate pain. In some cases, the swelling may initially worsen before it begins to improve, as the body responds to the infection and the treatment.

However, if you notice that the swelling is significantly increasing or if the pain is becoming unbearable, it is essential to follow up with your healthcare provider. There are several reasons why the swelling might persist or worsen despite antibiotic treatment:
1. Inadequate Antibiotic Coverage: The antibiotic prescribed may not be effective against the specific bacteria causing your epididymitis. It is crucial that the antibiotic is appropriate for the type of infection you have. If the bacteria are resistant to the antibiotic, the treatment will not be effective.

2. Abscess Formation: In some cases, an infection can lead to the formation of an abscess, which is a collection of pus that can cause increased swelling and pain. If an abscess develops, it may require drainage in addition to antibiotic therapy.

3. Underlying Conditions: There may be other underlying conditions contributing to the swelling and pain, such as testicular torsion or a hernia, which would require different management.

4. Delayed Response: As mentioned earlier, it can take time for antibiotics to take effect. If you have only been on the medication for a short period, it may be too early to assess its effectiveness fully.

5. Inadequate Pain Management: Sometimes, the pain associated with epididymitis can be severe, and additional pain management strategies may be necessary to help alleviate discomfort while the antibiotics take effect.

In conclusion, while some swelling and discomfort can be expected initially after starting antibiotics for epididymitis, significant worsening of symptoms should not be ignored. It is advisable to contact your healthcare provider to discuss your symptoms, as they may want to reassess your condition, possibly perform additional tests, or adjust your treatment plan. Early intervention can help prevent complications and ensure a more effective recovery. Always prioritize clear communication with your healthcare provider regarding your symptoms and any concerns you may have.

I recently developed epididymitis, and although the epididymis is swollen, it is not hard. After taking antibiotics for a few days, I noticed that the entire epididymis feels harder (not just locally). Is this normal? (I took Pseudomonas aeruginosa tablets 400 mg.)
