Phimosis
My son is currently two years old.
He underwent a circumcision twenty days after birth at the suggestion of his sister (as newborn males in the U.S.
are routinely circumcised by doctors).
The procedure was performed by the attending physician.
Now, at two years old, his glans is completely exposed, and during erections, the remaining foreskin is tightly pulled.
Does he need any corrective surgery, or will this condition have any physiological effects on his development in the future? I am very concerned.
Please provide me with a detailed answer.
Thank you.

There is still much debate regarding whether newborns should undergo circumcision.
The United States is the country that performs this procedure the most in the world.
Once circumcised, the glans will be exposed, making it easier to clean.
As for the current tightness issue, there is no need to rush into treatment; it may improve naturally after puberty when development is complete.
If there are still concerns, please take your son to see a urologist for evaluation.

Concerns regarding the frenulum in circumcised toddlers, particularly in your case where your two-year-old son has a tight frenulum that causes discomfort during erections, are valid and warrant careful consideration. The frenulum is a band of tissue on the underside of the penis that connects the glans (the head) to the shaft. In circumcised boys, the frenulum can sometimes be left intact, and if it is short or tight, it may lead to discomfort or pain during erections.
Understanding the Situation
1. Circumcision and Frenulum: In many circumcision procedures, especially those performed shortly after birth, the frenulum may not be fully addressed. This can lead to a situation where, although the foreskin is removed, the frenulum remains short and tight. This is often referred to as a "short frenulum" or "frenulum breve."
2. Symptoms and Concerns: If your son is experiencing tightness or discomfort during erections, it may indicate that the frenulum is indeed too short. This can lead to pain, difficulty with erections, or even tearing of the frenulum, which can be quite painful.
3. Potential Impact on Growth and Development: While a short frenulum does not typically cause long-term physical health issues, it can affect comfort and sexual function later in life. If left unaddressed, it may lead to psychological discomfort or anxiety regarding sexual health as he matures.
Recommendations
1. Consult a Pediatric Urologist: The best course of action is to consult a pediatric urologist. They specialize in urinary and reproductive health in children and can provide a thorough examination. They will assess the tightness of the frenulum and determine if it is causing any functional issues.
2. Possible Treatments: If the pediatric urologist determines that the frenulum is indeed too short and causing discomfort, they may recommend a simple surgical procedure called a frenuloplasty. This procedure involves lengthening the frenulum and is typically quick, with minimal recovery time.
3. Monitoring: If your son is not currently experiencing significant pain or issues, the doctor may suggest monitoring the situation as he grows. Sometimes, as children develop, the tightness may become less of an issue.
4. Education and Support: As a parent, it’s important to educate yourself about normal penile development and what to expect as your son grows. This knowledge can help alleviate some of your concerns and prepare you for discussions with healthcare providers.
Conclusion
It’s understandable to feel concerned about your son’s health and comfort. A tight frenulum in a circumcised toddler can be a common issue, but it is manageable. Consulting with a pediatric urologist will provide you with the best guidance tailored to your son’s specific situation. They can help determine if any intervention is necessary and ensure that your son remains healthy and comfortable as he grows. Remember, addressing these concerns early can prevent potential discomfort in the future and promote a positive outlook on his sexual health.
