Hair loss medications
Hello, I would like to ask about my situation.
I have been experiencing alopecia areata since December of last year, and I discovered I have polycystic ovary syndrome (PCOS) and irregular menstrual cycles in March of this year.
These issues started to appear gradually after giving birth.
I have been receiving monthly steroid treatments for my scalp.
My gynecologist suggested that I take Diane for a few months, but my dermatologist recommended that I take oral steroids because my hair continues to fall out.
Can I take these two medications together?
Niming, 20~29 year old female. Ask Date: 2017/04/10
Dr. Huang Yiwen reply Obstetrics and Gynecology
Hello, Miss Anonymous: It is not recommended to take both medications simultaneously; you should start with either one.
After all, there is no specific or effective treatment for alopecia areata.
However, some patients may naturally recover after a period of time.

When managing hair loss, particularly in the context of conditions like alopecia areata and polycystic ovary syndrome (PCOS), it's essential to approach treatment with caution and under the guidance of healthcare professionals. In your case, you are considering the simultaneous use of steroids for your scalp and Diane (a combination of estrogen and progestin) for PCOS.
First, let's clarify the roles of these medications. Steroids, particularly corticosteroids, are often used to reduce inflammation and suppress the immune response, which can be beneficial in treating alopecia areata. This condition is characterized by sudden hair loss in patches and is thought to be an autoimmune disorder. On the other hand, Diane is commonly prescribed for managing symptoms of PCOS, which can include irregular menstrual cycles, acne, and hair loss due to hormonal imbalances.
Combining these two treatments can be complex. While there is no absolute contraindication to using both medications together, it is crucial to consider the potential side effects and interactions. Corticosteroids can have systemic effects, especially if taken orally or in high doses, including weight gain, mood changes, and increased risk of infections. Diane, while generally well-tolerated, can also have side effects such as nausea, headaches, and an increased risk of blood clots, particularly in women who smoke or have other risk factors.
Given these considerations, it is advisable to consult with your healthcare providers before starting both treatments simultaneously. They can evaluate your overall health, the severity of your hair loss, and any other underlying conditions you may have. Your doctors may suggest starting with one treatment and monitoring your response before introducing the other. This approach allows for better management of side effects and a clearer understanding of which treatment is more effective for your specific situation.
In addition to medication, lifestyle changes can also play a significant role in managing hair loss and PCOS. Maintaining a balanced diet, regular exercise, and managing stress can help improve overall health and potentially mitigate some symptoms associated with both conditions. For instance, a diet rich in antioxidants, vitamins, and minerals can support hair health, while weight management can help regulate hormonal levels in women with PCOS.
Furthermore, it’s important to have realistic expectations regarding hair regrowth and treatment outcomes. Conditions like alopecia areata can sometimes resolve spontaneously, while PCOS may require ongoing management to control symptoms effectively. Regular follow-ups with your healthcare providers will help track your progress and make necessary adjustments to your treatment plan.
In conclusion, while combining steroids and Diane for managing hair loss related to PCOS may be possible, it is essential to do so under medical supervision. Your healthcare providers can help you navigate the complexities of these treatments, ensuring that you receive the most effective and safe care tailored to your needs. Always prioritize open communication with your doctors about your symptoms, treatment goals, and any concerns you may have.
