Visual field defect?
Hello, doctor.
First, let me explain my personal situation.
I have been nearsighted for a long time, with both eyes having a prescription of -9.00 diopters.
I undergo dilated fundus examinations every six months, with the most recent one in March, which was normal.
My question is that I have noticed when I look at a "whole" tiled wall with one eye, the left eye sees a distortion in the left part of the visual field, and the right eye sees a distortion in the right part of the visual field.
If I point my finger at that location, I can also feel that my finger appears distorted and blurry.
However, when I check with an Amsler grid, I do not notice this distortion, or the area of distortion is outside the range of the grid.
Since my fundus examinations have all been normal and I have no issues when using both eyes to see, this situation does not affect my daily life, but it does make me feel a bit uneasy.
I am unsure if I should pay attention to anything in particular.
Doctor, how should I approach this situation? Should I continue to self-monitor, or should I take any specific actions?

It's understandable to feel anxious about visual distortions, especially when you have a history of high myopia. Your description of experiencing distortion in your peripheral vision when looking at a flat surface, such as a tiled wall, raises some important considerations.
First, it's reassuring that your recent eye examinations, including dilated fundus exams, have returned normal results. This suggests that there are no immediate signs of retinal detachment or other serious ocular conditions. However, the experience of visual distortion can still be concerning and warrants further attention.
Visual distortions can arise from various factors, including refractive errors, retinal issues, or even neurological conditions. In your case, since you have high myopia, the shape of your eyeball may contribute to how light is focused on your retina, potentially leading to some visual anomalies. Additionally, the brain's processing of visual information can sometimes lead to perceived distortions, especially if there is a difference in how each eye is functioning.
Given that you are able to see normally with both eyes and that your vision is stable during routine activities, it may be appropriate to monitor the situation closely. Here are some steps you can take:
1. Self-Monitoring: Keep a journal of when these distortions occur, their duration, and any associated symptoms (like headaches or eye strain). This information can be valuable for your eye care provider.
2. Regular Eye Exams: Continue with your routine eye examinations, ideally every six months, as you have been doing. If you notice any changes in your vision or if the distortions become more frequent or severe, schedule an appointment sooner.
3. Visual Field Testing: If you haven't already, consider asking your eye care provider about a visual field test. This can help assess your peripheral vision more comprehensively and rule out any significant issues.
4. Consult a Specialist: If your symptoms persist or worsen, it may be beneficial to consult a retinal specialist or a neuro-ophthalmologist. They can provide a more in-depth evaluation of your visual system and determine if further testing is necessary.
5. Eye Health Maintenance: Maintain good eye health by following the 20-20-20 rule when using screens: every 20 minutes, look at something 20 feet away for at least 20 seconds. This can help reduce eye strain.
6. Lifestyle Considerations: Ensure you are managing any underlying health conditions, such as hypertension or diabetes, which can affect eye health. A balanced diet rich in antioxidants, omega-3 fatty acids, and vitamins can also support eye health.
7. Mental Health: It's important to address any anxiety or stress related to your vision. Consider speaking with a mental health professional if you find that your worries about your eyesight are affecting your daily life.
In summary, while your current situation does not seem to indicate an urgent problem, staying vigilant and maintaining regular check-ups is crucial. If you experience any new symptoms or changes, do not hesitate to seek medical advice. Your proactive approach to eye care is commendable, and with continued monitoring and appropriate care, you can manage your visual health effectively.
