Due to premature ejaculation, taking Clomipramine?
Hello, doctor.
I have premature ejaculation, and based on the recommendation of a pharmacist at a regular pharmacy, I took Clomipramine.
The pharmacist mentioned that Clomipramine is a temporary solution, and I took one tablet three hours before sexual activity, thinking it would extend my duration from just a few seconds to three minutes as the pharmacist suggested.
However, after taking one tablet, I still only lasted a few seconds before ejaculating.
In this situation, can I take two tablets of Clomipramine at once? Would the effect be stronger?

The only approved medication for premature ejaculation is "Priligy." It is recommended to seek treatment from a qualified physician.
It is not advisable to purchase medication from a pharmacy for treatment without a doctor's diagnosis and evaluation.

St. John's Wort, known scientifically as Hypericum perforatum, is a herbal remedy commonly used for its antidepressant properties. However, its efficacy in treating premature ejaculation (PE) is not well-established in clinical literature. While some individuals may seek alternative treatments for PE, it is essential to approach this condition with a comprehensive understanding of its causes and available treatment options.
Premature ejaculation is a common sexual dysfunction characterized by ejaculation that occurs with minimal sexual stimulation, often before the individual wishes. The causes of PE can be multifactorial, including psychological factors (such as anxiety or stress), biological factors (such as hormonal imbalances or neurological issues), and relational factors (such as lack of communication with a partner). Therefore, treatment should ideally address the underlying causes rather than relying solely on herbal supplements.
Regarding the use of St. John's Wort, it is crucial to note that this herb is primarily used for mood disorders, particularly mild to moderate depression. Its mechanism of action involves the inhibition of the reuptake of neurotransmitters such as serotonin, dopamine, and norepinephrine. While some anecdotal evidence suggests that St. John's Wort may have a calming effect, which could indirectly help with performance anxiety related to PE, there is no scientific evidence supporting its direct efficacy in prolonging ejaculation.
Taking two St. John's Wort capsules instead of one may seem like a logical approach to enhance its effects; however, this is not advisable without consulting a healthcare professional. Increasing the dosage of any supplement can lead to potential side effects or interactions with other medications. St. John's Wort is known to interact with various medications, including antidepressants, anticoagulants, and oral contraceptives, potentially reducing their effectiveness or increasing the risk of side effects.
For individuals experiencing premature ejaculation, several evidence-based treatment options are available. These include:
1. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help individuals gain better control over ejaculation.
2. Topical Anesthetics: Creams or sprays containing numbing agents can be applied to the penis to reduce sensitivity and delay ejaculation.
3. Medications: Certain prescription medications, such as selective serotonin reuptake inhibitors (SSRIs), have been shown to delay ejaculation as a side effect. Dapoxetine is a medication specifically designed for the treatment of PE and is available in some countries.
4. Counseling or Therapy: If psychological factors contribute to PE, speaking with a therapist or counselor may help address anxiety or relationship issues.
5. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can improve control over ejaculation.
In conclusion, while St. John's Wort may have some benefits for mood enhancement, its role in treating premature ejaculation is not supported by scientific evidence. It is essential to consult with a healthcare provider to explore appropriate treatment options tailored to individual needs. Self-medicating with herbal supplements without professional guidance can lead to unintended consequences, and a healthcare provider can help determine the best course of action for managing premature ejaculation effectively.
