Itching in the genital area before menstruation?
Hello Doctor, since January of this year, I have been experiencing itching in the genital area one month before my menstrual period.
Sometimes, the itching extends to the area around the anus, and it eventually becomes both itchy and painful, with significant swelling of the labia.
I usually use panty liners, and during the week before my period, there is a considerable amount of yellow discharge on the liner.
When I urinate, there is also a thick, clear fluid that is secreted from the vagina and floats on the surface of the water.
I would like to ask if this is normal and if there are ways to improve or alleviate this condition.
If I visit a gynecologist, will an internal examination be necessary? Thank you, Doctor.

Understanding itchy genital symptoms before menstruation can be quite complex, as various factors may contribute to this discomfort. Based on your description, it seems that you are experiencing significant itching, swelling, and unusual discharge in the days leading up to your menstrual cycle. This is not uncommon, and several potential causes could be at play.
One of the most common reasons for increased itching and irritation in the genital area before menstruation is hormonal fluctuations. The menstrual cycle involves significant changes in hormone levels, particularly estrogen and progesterone. These hormonal shifts can lead to changes in the vaginal environment, including alterations in pH and moisture levels, which may cause irritation and increased susceptibility to infections.
Another potential cause could be a yeast infection, which is characterized by itching, swelling, and a thick, often yellowish discharge. Yeast infections can be exacerbated by hormonal changes, especially during the luteal phase of the menstrual cycle (the time between ovulation and the start of menstruation). The presence of a yellow discharge and the itching you describe could suggest a yeast infection or possibly bacterial vaginosis, which is another common condition that can lead to similar symptoms.
Additionally, contact dermatitis could be a factor. This condition occurs when the skin reacts to irritants or allergens, such as those found in scented hygiene products, soaps, or even the materials in panty liners. If you have recently changed any products you use in the genital area, this could be contributing to your symptoms.
Regarding your question about whether this is normal, it is essential to understand that while some women may experience mild discomfort or changes in discharge before their period, the severity of your symptoms suggests that it would be wise to seek medical advice. It is not typical for the symptoms to be as pronounced as you describe, especially if they are causing pain and significant discomfort.
To address your symptoms, here are some recommendations:
1. Maintain Hygiene: Ensure that you are practicing good hygiene, but avoid over-washing the area, which can strip natural oils and lead to further irritation.
2. Avoid Irritants: Consider switching to unscented products and avoiding any potential irritants, such as scented pads or soaps.
3. Over-the-Counter Treatments: If you suspect a yeast infection, antifungal treatments are available over the counter. However, it is advisable to consult with a healthcare provider before starting any treatment.
4. Diet and Lifestyle: Maintaining a balanced diet and staying hydrated can help support your overall health and may reduce the frequency of infections.
5. Consult a Healthcare Provider: Given the severity of your symptoms, it is advisable to see a gynecologist. During your visit, an internal examination may be necessary to assess the situation accurately. This examination can help rule out infections, assess for any abnormalities, and provide a tailored treatment plan.
In summary, while some changes in the genital area before menstruation can be normal, the symptoms you are experiencing seem to warrant further investigation. A healthcare provider can help identify the underlying cause and recommend appropriate treatments to alleviate your discomfort. Don't hesitate to seek medical advice, as addressing these symptoms early can lead to better outcomes and improved quality of life.
