Anal itching
The itching around the anus has persisted for several years, occurring at unpredictable times—sometimes at night and sometimes during the day.
It flares up occasionally, but it is not a daily occurrence.
However, after each bowel movement, when wiping the anus with toilet paper, there are several small, white, elongated objects on the paper.
I am unsure if these are pinworms.

Based on your description, these white foreign objects could indeed be parasites.
It is recommended that you visit a hospital outpatient clinic for further examination.

Anal itching, or pruritus ani, is a common condition that can be caused by a variety of factors. Given your description of the symptoms persisting for several years, with intermittent episodes of itching, it is important to consider both common and less common causes.
One of the most frequent causes of anal itching is poor hygiene or excessive cleaning. Inadequate cleaning after bowel movements can lead to irritation, while excessive cleaning can strip the skin of its natural oils, leading to dryness and itching. Additionally, the use of scented soaps or wipes can exacerbate irritation.
Another potential cause is the presence of pinworms (Enterobius vermicularis), especially if you notice white, elongated objects on the toilet paper after wiping. Pinworms are a common parasitic infection, particularly in children, but they can affect adults as well. The female pinworm lays eggs around the anal area, leading to intense itching, especially at night. If you suspect pinworms, a stool test or a tape test can be performed to confirm their presence.
Other possible causes of anal itching include:
1. Skin Conditions: Conditions such as eczema, psoriasis, or contact dermatitis can lead to itching around the anal area. These conditions may require topical treatments or lifestyle modifications to manage symptoms.
2. Infections: Fungal infections, such as candidiasis, or bacterial infections can also cause anal itching. These infections may be more common in individuals with diabetes or those who are immunocompromised.
3. Dietary Factors: Certain foods, such as spicy foods, caffeine, and alcohol, can irritate the anal area and lead to itching. Keeping a food diary may help identify any dietary triggers.
4. Hemorrhoids: Swollen blood vessels in the rectal area can cause discomfort and itching. Hemorrhoids may be internal or external and can be associated with other symptoms such as bleeding or pain.
5. Hygiene Products: The use of scented toilet paper, soaps, or lotions can irritate the sensitive skin around the anus, leading to itching.
Given the chronic nature of your symptoms, it is advisable to seek medical attention. A healthcare provider can perform a thorough examination, which may include a visual inspection and possibly a stool test to check for pinworms or other infections. They may also inquire about your hygiene practices, diet, and any other symptoms you may be experiencing.
In the meantime, here are some general recommendations to alleviate anal itching:
- Maintain Good Hygiene: Clean the anal area gently with mild soap and water. Avoid using harsh soaps or scented products.
- Avoid Scratching: Scratching can worsen irritation and lead to secondary infections.
- Use Moisturizers: If dryness is an issue, consider applying a gentle, fragrance-free moisturizer to the area.
- Wear Loose Clothing: Tight clothing can trap moisture and heat, exacerbating itching. Opt for breathable fabrics.
- Consider Dietary Changes: If you suspect certain foods may be triggering your symptoms, try eliminating them from your diet to see if there is an improvement.
In conclusion, while anal itching can be a benign issue, it can also indicate underlying conditions that require treatment. Given your symptoms and the presence of potential pinworms, it is important to consult a healthcare professional for a proper diagnosis and treatment plan.
