Contract scabies?

Hello, doctor.
I visited a dermatologist in early April and was diagnosed with scabies.
I was prescribed a topical ointment for full-body application.
However, the redness and swelling on my genitals have subsided and then returned.
I have also treated my clothes with hot water.
Is this a sign of incomplete treatment? Does bathing with sulfur water kill scabies? Should I frequently wash my bed linens in hot water?

Scabies, caused by the Sarcoptes scabiei mite, is a highly contagious skin condition characterized by intense itching and a rash. Treatment typically involves the use of topical scabicides, such as permethrin cream or benzyl benzoate, which are applied to the entire body from the neck down and left on for a specified period before washing off. It’s crucial to follow the treatment regimen precisely to ensure complete eradication of the mites.

In your case, it seems that you have already been treated with a topical medication, which is a standard approach. However, the persistence of symptoms, such as redness and swelling in the genital area, may indicate that the treatment was not fully effective or that there may be a secondary infection or irritation. It’s essential to consult your healthcare provider if symptoms persist after treatment, as they may need to reassess your condition and possibly prescribe a different medication or additional treatments.

Regarding your question about sulfur baths, sulfur has been traditionally used in various forms to treat scabies due to its insecticidal properties. However, while sulfur can help in some cases, it is not a primary treatment for scabies. The most effective treatments remain the topical scabicides mentioned earlier. If you are considering using sulfur, it is advisable to discuss this with your healthcare provider first.

As for cleaning your environment, it is crucial to wash all clothing, bedding, and towels in hot water (at least 130°F or 54°C) and dry them on a hot cycle to kill any mites that may be present. Items that cannot be washed should be sealed in a plastic bag for at least 72 hours, as mites cannot survive without a human host for more than a few days. Regular cleaning of your living space and personal items is essential to prevent reinfestation and to protect others from contracting scabies.

In summary, here are some tips for complete recovery from scabies:
1. Follow Treatment Instructions: Ensure you apply the prescribed medication correctly and for the full duration recommended by your healthcare provider.

2. Consult Your Doctor: If symptoms persist or worsen, return to your healthcare provider for further evaluation and possible alternative treatments.

3. Environmental Hygiene: Wash all clothing, bedding, and towels in hot water and dry them on a hot cycle. Seal non-washable items in plastic bags for at least 72 hours.

4. Avoid Close Contact: Until you are confirmed to be free of scabies, avoid close contact with others to prevent spreading the mites.

5. Monitor Symptoms: Keep an eye on any new symptoms or changes in your condition, and report these to your healthcare provider.

By adhering to these guidelines and maintaining open communication with your healthcare provider, you can work towards a complete recovery from scabies.
