Infected with scabies for two months, leading to depression during treatment?
I have been infected with scabies for two months, and my treatment involves applying permethrin (hereafter referred to as "P cream") and using a scabicide.
Every week, I send a set of four bed linens to a laundromat where they are washed with bleach and laundry detergent, then dried at high temperatures.
To treat my scabies, I spent 30,000 NT to update my personal washing machine and separate my laundry from my family’s.
I also wash my regular clothes in hot water and dry them.
I apply medications such as Meikang, Keli Shu, and Jianlian twice daily, using the same method as the P cream, applying it all over my body below the neck.
There are also many nodules in the scrotal area.
After two months of this treatment, there has been no improvement, and I am feeling increasingly fatigued both mentally and physically, with poor appetite.
I have lost 5 kilograms in these two months.
I would like to ask if there are more specialized dermatologists in the Tainan area who can offer surgical options or injectable medications for a quicker resolution of my scabies issue.
Additionally, can I apply the P cream every three days? The clinic I visited recommended applying it once a week, but I want to expedite my recovery and am considering increasing the dosage or exploring other better treatment options.

A clinical assessment may need to be conducted first before recommending medication.

Dealing with scabies can be incredibly challenging, both physically and emotionally. Scabies is caused by a mite called Sarcoptes scabiei, which burrows into the skin, leading to intense itching and discomfort. Your current treatment regimen, which includes permethrin cream (often referred to as "P cream") and other topical medications, is a standard approach for managing scabies. However, it seems that despite your diligent efforts, you have not seen significant improvement over the past two months.
First, it's essential to understand that scabies treatment typically requires not just topical medications but also thorough cleaning of the environment. You mentioned that you have been washing your bedding and clothing with bleach and high heat, which is excellent. This step is crucial because scabies mites can survive for a short time off the human body, and any untreated items can lead to reinfestation.
Regarding your question about the frequency of applying permethrin cream, the standard recommendation is to apply it once and then repeat the application after one week. This is to ensure that any newly hatched mites are also killed. Applying it more frequently than recommended, such as every three days, is not advised without medical supervision, as it may lead to skin irritation or other side effects without providing additional benefits.
If you are not seeing improvement, it may be time to consult a dermatologist who specializes in skin conditions, particularly in the Tainan area as you mentioned. A dermatologist can evaluate your condition more thoroughly and may consider alternative treatments. For instance, oral medications like ivermectin are sometimes used for scabies, especially in cases that are resistant to topical treatments. This medication can be particularly effective in severe cases or when there is a widespread infestation.
Additionally, it’s crucial to address the emotional and psychological toll that this condition can take. The stress, anxiety, and physical discomfort can lead to a decline in mental health, as you've noted with your decreased appetite and weight loss. Seeking support from mental health professionals or support groups can be beneficial during this challenging time.
In summary, while your current treatment with permethrin is appropriate, the lack of improvement after two months suggests that you may need a reevaluation of your treatment plan. Consulting with a specialized dermatologist can provide you with more tailored options, including the possibility of oral medications. Remember to continue with environmental cleaning and consider seeking psychological support to help manage the emotional aspects of dealing with scabies. Your health and well-being are paramount, and there are effective treatments available that can help you overcome this condition.
