Do I need to undergo another gastroscopy?
Hello doctor, I had an upper gastrointestinal endoscopy in early January which revealed gastritis and gastroesophageal reflux disease (GERD).
My condition has improved significantly with medication, but recently I have been experiencing occasional stomach pain and bloating, and I have a reduced appetite.
Taking medication helps a lot.
Do you think I need to undergo another endoscopy? I am very worried about the possibility of stomach cancer, especially since I am preparing for a civil service exam in July, which is making me quite anxious and stressed.

Hello: It is important to pay attention to diet and emotional adjustments.
Check-ups and medication are not the best solutions unless symptoms persist and worsen.
In that case, you may consult a gastroenterologist for further evaluation.

It's understandable to feel anxious about ongoing stomach issues, especially when preparing for something as significant as a civil service exam. Based on your previous endoscopy results, which indicated gastritis and gastroesophageal reflux disease (GERD), it seems that your symptoms have improved with medication. However, the recurrence of stomach pain and bloating, along with decreased appetite, raises valid concerns.
First, let's address the necessity of another endoscopy. Generally, if your symptoms have improved significantly with medication, and you are not experiencing alarming symptoms such as severe weight loss, persistent vomiting, or blood in your stool, it may not be immediately necessary to undergo another endoscopy. However, if your symptoms persist or worsen, or if you develop new symptoms, it would be prudent to consult your healthcare provider about the possibility of another endoscopy. This procedure can provide a clearer picture of your gastric health and rule out any serious conditions, including gastric cancer.
It's also important to consider the psychological aspect of your situation. Stress and anxiety can exacerbate gastrointestinal symptoms. The pressure of preparing for exams can lead to increased stomach discomfort, which may not necessarily be related to a physical ailment. Stress can lead to increased acid production and muscle tension in the gastrointestinal tract, contributing to symptoms like bloating and pain. Techniques such as mindfulness, relaxation exercises, or even talking to a counselor may help alleviate some of this stress and, in turn, improve your gastrointestinal symptoms.
In terms of your concerns about gastric cancer, while it's natural to worry, it's essential to remember that the majority of gastric issues are not cancerous. Your previous endoscopy results showed no signs of malignancy, and the presence of gastritis and GERD is more common and manageable. Regular follow-ups with your healthcare provider can help monitor your condition and provide peace of mind.
If you decide to pursue further evaluation, your doctor may recommend additional tests, such as an ultrasound or a CT scan, to assess your stomach and surrounding organs without the need for invasive procedures. Additionally, if your symptoms are primarily related to GERD, lifestyle modifications such as dietary changes, weight management, and avoiding triggers like spicy or fatty foods can also be beneficial.
In summary, while another endoscopy may not be immediately necessary, it is crucial to maintain open communication with your healthcare provider about your symptoms. They can help determine the best course of action based on your current condition and any changes you experience. Remember to take care of your mental health during this stressful time, as it can significantly impact your physical well-being.
