Surgery and its impact on the gastrointestinal system?
I just had surgery for an aneurysm last month, during which part of my liver and kidney were removed.
However, I now find myself easily experiencing constipation from consuming hot and dry foods, and diarrhea from cold and cool foods.
Is this due to my gastrointestinal system becoming too weak after the surgery? Thank you for your response, doctor.

Dr. Yan Sumei reply Traditional Chinese Medicine
Dear online friend Tao Miao: Thank you for your letter.
I agree with your perspective.
It has only been a month since your surgery, and your body’s energy and blood have not fully recovered yet, which is why you are experiencing this.
After some time of proper care, your gastrointestinal function will improve.
Wishing you health and peace! Sincerely, Yan Su-Mei, Department of Traditional Chinese Medicine, Nantou Hospital.

More Info
Post-surgery digestive issues are quite common, especially after major abdominal surgeries such as the one you described involving the removal of parts of the liver and kidney. The gastrointestinal (GI) system is sensitive and can react in various ways to surgical interventions, leading to symptoms like constipation and diarrhea, as you've experienced.
Understanding Gastrointestinal Changes After Surgery
1. Altered Gut Motility: After surgery, the normal motility of the intestines can be disrupted. This can lead to conditions such as ileus, where the bowel temporarily stops moving. This can result in constipation or difficulty passing stool. Conversely, some patients may experience diarrhea due to rapid transit through the intestines, especially if the gut flora has been altered or if there is a change in diet post-surgery.
2. Dietary Sensitivities: Your observation that certain foods affect your bowel movements is significant. After surgery, the digestive system may become more sensitive to specific types of food. Foods that are spicy, greasy, or high in fiber can exacerbate constipation, while cold or raw foods might trigger diarrhea. It’s essential to monitor your diet and identify which foods cause discomfort or irregular bowel movements.
3. Changes in Gut Flora: Surgery can alter the balance of bacteria in the gut, which plays a crucial role in digestion and overall gut health. This imbalance can lead to digestive issues, including bloating, gas, constipation, or diarrhea. Probiotics may help restore this balance, but it’s best to consult with your healthcare provider before starting any supplements.
4. Hydration and Fiber Intake: Post-surgery, it’s crucial to maintain adequate hydration and a balanced diet rich in fiber to support healthy bowel movements. If you’re experiencing constipation, increasing your water intake and consuming fiber-rich foods like fruits, vegetables, and whole grains can help. However, if you find that fiber exacerbates your symptoms, you may need to adjust your intake accordingly.
5. Medication Effects: If you were prescribed pain medications or other drugs post-surgery, these could also contribute to constipation. Opioids, in particular, are known to slow down gut motility. Discussing your medication regimen with your doctor can help identify if any adjustments are necessary.
6. Physical Activity: Engaging in light physical activity, as permitted by your healthcare provider, can stimulate bowel function and help alleviate constipation. Gentle walking can be beneficial in promoting gut motility.
7. Follow-Up Care: It’s crucial to maintain regular follow-ups with your healthcare provider to monitor your recovery. If digestive issues persist or worsen, further evaluation may be necessary to rule out complications such as bowel obstruction or infection.
Conclusion
In summary, the digestive issues you are experiencing post-surgery are likely related to the surgical intervention and the subsequent changes in your gastrointestinal function. It’s essential to pay attention to your diet, stay hydrated, and maintain communication with your healthcare provider regarding your symptoms. If your digestive issues continue or significantly impact your quality of life, further investigation may be warranted to ensure there are no underlying complications. Always consult your physician before making any significant changes to your diet or lifestyle post-surgery.
