Pain in the right flank and right testicle?
Hello Doctor: Yesterday, I exerted a bit while having a bowel movement, but it wasn't constipation.
Afterward, I suddenly felt a slight discomfort and tightness in my right testicle.
I have previously been diagnosed with varicocele in both testicles.
I thought I would ignore it and see if it would improve after sleeping, but by this afternoon, my right lower back is still quite sore, and the right testicle has a slight aching and swelling sensation.
Could this be a symptom of varicocele, or is there a possibility of a hernia? Thank you for taking the time to respond.

Hello: You may have varicose veins or a hernia, but the likelihood of having both conditions is low.
It is recommended that if you do not have any other symptoms, you can observe for a few days.
If you experience any additional symptoms, please seek medical attention.
Wishing you good health.

Hello,
Thank you for reaching out with your concerns regarding the discomfort you're experiencing in your right testicle and lower back. Based on your description, it sounds like you may be dealing with a few potential issues, including the possibility of varicocele, which you mentioned you have been previously diagnosed with, or even a hernia.
Varicocele is a condition characterized by enlarged veins within the scrotum, similar to varicose veins that can occur in the legs. This condition can lead to a feeling of heaviness or discomfort in the affected testicle, and it is not uncommon for individuals with varicocele to experience pain that can radiate to the lower back or groin area. The discomfort you felt after straining during a bowel movement could have exacerbated this condition, leading to the symptoms you're currently experiencing.
On the other hand, the symptoms you describe could also suggest the presence of an inguinal hernia. An inguinal hernia occurs when a portion of the intestine protrudes through a weak spot in the abdominal muscles, often leading to discomfort or pain in the groin or scrotal area. The fact that you felt a tight sensation in your right testicle after straining could indicate that a hernia is present, especially if the pain persists or worsens with certain movements or activities.
Given that you are experiencing persistent discomfort in both the right testicle and lower back, it is essential to seek medical attention. A healthcare provider can perform a physical examination and may recommend imaging studies, such as an ultrasound, to assess the condition of your testicles and the surrounding structures. This will help determine whether your symptoms are related to varicocele, a hernia, or another underlying issue.
In the meantime, here are a few recommendations to help manage your discomfort:
1. Avoid Straining: Try to avoid activities that require heavy lifting or straining, as these can worsen your symptoms. If you are experiencing constipation, consider dietary changes to include more fiber and stay hydrated to promote regular bowel movements.
2. Pain Relief: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help alleviate discomfort. However, be sure to follow the recommended dosages and consult with a healthcare provider if you have any underlying health conditions or are taking other medications.
3. Supportive Underwear: Wearing supportive underwear can help reduce discomfort by providing support to the scrotum and minimizing movement.
4. Monitor Symptoms: Keep track of any changes in your symptoms, including the intensity of pain, any swelling, or changes in the appearance of the scrotum. This information will be valuable for your healthcare provider.
5. Seek Medical Attention: If your symptoms persist, worsen, or if you experience additional symptoms such as swelling, redness, or fever, it is crucial to seek medical attention promptly. These could be signs of a more serious condition that requires immediate intervention.
In conclusion, while varicocele and hernia are two potential causes of your symptoms, a thorough evaluation by a healthcare professional is necessary to determine the exact cause and appropriate treatment. Please do not hesitate to reach out to a doctor for further evaluation and management of your symptoms. Your health and well-being are important, and addressing these concerns early can help prevent complications.
Take care, and I wish you a speedy recovery.
