Phimosis during sexual intercourse?

Hello doctor, I am a freshman in high school and I have phimosis.
My question is that I can pull the foreskin back behind the glans, but the glans is very sensitive to even slight contact.
So I would like to ask, when I want to have sex in the future, should I pull the glans back or just clean the smegma and let the glans protrude a little before starting?

Niming, 10~19 year old female. Ask Date: 2017/07/13

Dr. Du Shixi reply Urology

Hello: If phimosis can be retracted, it should be retracted daily for cleaning, dried, and then pushed back without the need for special soap or body wash.
Over time, it will gradually become less sensitive, and during intercourse, the glans will naturally be exposed when entering the vagina.
Wishing you good health.

Managing phimosis, especially during the teenage years, can be a sensitive topic, but it’s important to address it for both sexual health and comfort. Phimosis is a condition where the foreskin cannot be easily retracted over the glans (the head of the penis). While it is common in younger males, many will find that their foreskin becomes more retractable with age. However, if you are experiencing discomfort or sensitivity, it’s essential to approach the situation carefully.

Understanding Phimosis and Sensitivity
1. Sensitivity of the Glans: It’s normal for the glans to be sensitive, especially if it has been covered by the foreskin for a long time. This sensitivity may decrease over time as the glans becomes accustomed to exposure.
2. Hygiene: Maintaining good hygiene is crucial. Regularly cleaning under the foreskin can help prevent the buildup of smegma (a combination of dead skin cells and oils), which can lead to irritation or infection. Gently retracting the foreskin (if possible) during bathing and cleaning the area with mild soap and water can be beneficial.

3. Gradual Exposure: If you can retract your foreskin but find it uncomfortable, consider gradually exposing the glans to air and gentle stimulation. This can help desensitize the area over time.

Preparing for Sexual Activity
When it comes to sexual activity, here are some tips to consider:
1. Consult a Healthcare Professional: Before engaging in sexual activity, it’s advisable to consult a healthcare provider, such as a urologist. They can assess your specific situation and provide personalized advice. They may recommend treatments or techniques to help with phimosis if it is causing significant discomfort.

2. Use Lubrication: If you decide to engage in sexual activity, using a water-based lubricant can help reduce friction and discomfort. This is especially important if you are still experiencing sensitivity.

3. Communication with Partners: If you have a partner, open communication about your condition and comfort levels is essential. Discussing any concerns or preferences can help both partners feel more comfortable.

4. Consider the Position of the Foreskin: During sexual activity, some individuals with phimosis choose to keep the foreskin in place, while others may retract it. If you find that retracting the foreskin is uncomfortable, it’s perfectly acceptable to leave it in place. The key is to find what feels best for you.

5. Take Your Time: Engaging in sexual activity for the first time can be nerve-wracking. Take your time, and don’t rush the experience. Focus on what feels good and comfortable for both you and your partner.

When to Seek Further Help
If you experience pain, difficulty retracting the foreskin, or any signs of infection (such as redness, swelling, or discharge), it’s important to seek medical advice. In some cases, a healthcare provider may recommend treatments such as topical steroids to help with phimosis or, in more severe cases, surgical options like circumcision.

Conclusion
Managing phimosis involves a combination of good hygiene, gradual exposure, and open communication with partners. It’s essential to listen to your body and seek professional advice if you have concerns. Remember that sexual health is an important aspect of overall well-being, and taking steps to address any issues can lead to a more comfortable and enjoyable experience in the future.
