Ear problems

Recently, after cleaning my ears, my left ear has been feeling uncomfortable.
Whenever I open my mouth or yawn, it feels like there's something in my ear, and I can also hear a sound.
Could you please consult a doctor? Thank you.

Chen, 20~29 year old female. Ask Date: 2017/07/18

Dr. Ji Zhaoyun reply Otolaryngology

Hello Mr.
Chen,
The issues you mentioned may be due to: 1) Eustachian tube dysfunction: This is the most common cause and can occur not only during air travel, mountain climbing, or diving, but also when forcefully blowing your nose or bending over.
2) Degeneration of the auditory nerve.
3) Otitis media: a) Acute otitis media: This often occurs alongside upper respiratory infections (such as colds) and sinusitis, and in severe cases, there may be ear discharge.
b) Chronic otitis media with acute exacerbation: This usually occurs when there is a pre-existing perforation in the eardrum, which can be caused by contaminated water or improper ear cleaning, leading to inflammation.

All of the above conditions can be appropriately treated with medication through medical consultation.
If symptoms persist, it is advisable to seek medical evaluation and treatment.

Wishing you safety and good health,
Dr.
Chi Chao-Yun,
Director of the Department of Otolaryngology, Taichung Hospital, Ministry of Health and Welfare.

More Info

Experiencing discomfort in the ear after cleaning can be quite concerning, and it’s important to understand the potential causes and when to seek medical attention. Based on your description, it seems that you are experiencing a sensation of fullness or pressure in your left ear, accompanied by sounds when you open your mouth or yawn. This could be indicative of several conditions, including earwax impaction, Eustachian tube dysfunction, or even inflammation of the ear canal.

Understanding Ear Discomfort
1. Earwax Impaction: One of the most common reasons for discomfort after cleaning the ears is earwax buildup. While cleaning, it’s possible to push wax deeper into the ear canal, leading to blockage. This can cause a sensation of fullness, muffled hearing, and sometimes even pain.

2. Eustachian Tube Dysfunction: The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure. If this tube is not functioning properly, it can lead to a feeling of fullness in the ear, especially when yawning or swallowing. This dysfunction can occur due to allergies, colds, or sinus infections.

3. Injury or Inflammation: If the cleaning was too aggressive, it could have caused minor trauma to the ear canal or even the eardrum, leading to inflammation and discomfort. Symptoms may include pain, a feeling of fullness, and changes in hearing.

When to See a Doctor
- Persistent Symptoms: If the discomfort continues for more than a few days, it is advisable to see a healthcare professional. Persistent symptoms could indicate an underlying issue that requires treatment.

- Severe Pain or Discharge: If you experience severe pain, bleeding, or discharge from the ear, you should seek medical attention immediately. These symptoms could indicate an infection or more serious injury.

- Hearing Changes: If you notice significant changes in your hearing, such as a sudden decrease in hearing ability, it’s important to consult a doctor. This could be a sign of a more serious condition that needs evaluation.

Medical Evaluation
During your visit, a healthcare provider will likely perform an otoscopic examination to look inside your ear canal and assess for any blockages, inflammation, or signs of infection. They may also conduct a hearing test to evaluate your auditory function. Depending on the findings, treatment options may include:
- Earwax Removal: If earwax impaction is confirmed, the doctor may use specialized tools to safely remove the wax.

- Medication: If there is an infection or inflammation, the doctor may prescribe antibiotics or anti-inflammatory medications.

- Eustachian Tube Dysfunction Treatment: If the issue is related to Eustachian tube dysfunction, treatments may include nasal decongestants, antihistamines, or even specific maneuvers to help equalize ear pressure.

Conclusion
In summary, while some discomfort after ear cleaning can be normal, it’s essential to monitor your symptoms closely. If discomfort persists or worsens, or if you experience any alarming symptoms, do not hesitate to seek medical advice. Ear health is crucial, and timely intervention can prevent complications and ensure your auditory system remains healthy. Always remember that when it comes to ear cleaning, it’s best to avoid inserting objects into the ear canal, as this can lead to more harm than good. Regular check-ups with an ear, nose, and throat (ENT) specialist can help maintain ear health and address any concerns you may have.
