Side effects of steroid use?
Hello, pharmacist.
I would like to ask about the use of the steroid cream (Clobetasol) that I've been applying for two weeks, followed by a break of 4 to 5 days.
I have been using it for almost a week now, reducing the frequency to once a day (previously twice a day, morning and evening).
Currently, the skin on the glans is dry and appears wrinkled, almost like a layer of flaky skin.
Is this side effect serious or acceptable? I need to continue applying it until the symptoms disappear, but I'm unsure how much longer I should continue! Previously, I had broken skin, and the doctor prescribed an antibiotic ointment (Mupirocin) to use together.
Thank you!

1.
Ingredients of Xianlis: FLUMETHASONE PIVALATE; SALICYLIC ACID.
Salicylic acid has a keratolytic effect, which promotes the penetration of Flumethasone pivalate into the skin, making it suitable for use in severe keratinization disorders, tinea, and sclerotic skin lesions.
(1) Adding salicylic acid or urea can help dissolve the stratum corneum, allowing for better absorption of the medication into the skin to enhance its efficacy.
2.
In flexural areas such as the groin and armpits, due to higher temperatures and humidity, a sealing effect occurs that further increases the transdermal absorption of the medication.
Therefore, it is advisable to avoid using high-potency corticosteroids (Class III or IV).
Flumethasone is a lower-potency corticosteroid with fewer side effects.
The medication should be discontinued once the skin symptoms have completely resolved; it should not be used continuously.
3.
The most common side effects include vasodilation, which can lead to the appearance of atrophy striae.
Once the skin becomes thin, the dermis lacks protective tissue around the blood vessels, making it susceptible to injury and bleeding, resulting in ecchymosis.
Skin atrophy or striae may easily occur in the groin or armpits.
Some individuals may notice the appearance of surrounding hypopigmented areas when skin diseases are resolved with topical corticosteroids; this is due to pigment loss but usually resolves over time, so there is no need for concern.
Topical corticosteroids should not be used for more than three months, as the side effects are generally milder.
4.
If there is no improvement after one week, consult a physician.
Do not purchase dermatological medications without a prescription, as this may lead to adverse effects.
5.
Corticosteroid ointments can be used in conjunction with antibiotic ointments, but this should only be done with confirmation and a prescription from a physician.

More Info
When it comes to the use of topical steroid creams, such as the one you mentioned (仙麗絲, or Clobetasol), understanding their side effects and proper usage is crucial for effective treatment and minimizing potential complications. Topical steroids are commonly prescribed for various skin conditions, including eczema, psoriasis, and dermatitis, due to their anti-inflammatory properties. However, prolonged use can lead to several side effects, which you seem to be experiencing.
Side Effects of Topical Steroids
1. Skin Thinning (Atrophy): One of the most common side effects of long-term steroid use is skin thinning. This can manifest as a wrinkled appearance or a "shiny" look to the skin. In your case, the "wrinkled" appearance of the skin on your glans (龜頭) could be indicative of this effect. Skin atrophy can make the skin more susceptible to injury and infection.
2. Tachyphylaxis: This term refers to the phenomenon where the skin becomes less responsive to the medication over time, requiring higher doses or more frequent applications to achieve the same effect. This could explain why you feel the need to continue using the cream even after a period of cessation.
3. Perioral Dermatitis: This is a condition that can arise from the use of topical steroids, characterized by red, inflamed bumps around the mouth and sometimes the eyes. It’s essential to monitor for any new rashes or irritations.
4. Steroid Withdrawal: After stopping the use of topical steroids, some patients may experience a rebound effect where the original symptoms return, sometimes worse than before. This can lead to a cycle of overuse.
5. Systemic Absorption: Although rare, especially with topical applications, there is a risk of systemic absorption leading to side effects such as adrenal suppression, particularly if used on large areas of the body or under occlusive dressings.
Recommendations for Use
1. Follow Doctor's Instructions: It’s crucial to adhere to your healthcare provider's instructions regarding the frequency and duration of use. If your doctor has recommended tapering down from twice daily to once daily, it’s essential to follow that guidance.
2. Moisturize: Using a good moisturizer can help alleviate dryness and irritation. Apply the moisturizer after the steroid cream has been absorbed to help lock in moisture.
3. Monitor Symptoms: Keep a close eye on your symptoms. If you notice increased irritation, redness, or any new symptoms, it’s advisable to consult your healthcare provider.
4. Consider Alternatives: If you find that the steroid cream is not providing the relief you need or is causing side effects, discuss alternative treatments with your doctor. There are non-steroidal options available that may be effective for your condition.
5. Gradual Tapering: If you have been using the cream for an extended period, consider a gradual tapering approach rather than stopping abruptly. This can help minimize withdrawal symptoms and rebound flare-ups.
Conclusion
In summary, while topical steroids can be effective for managing skin conditions, they come with a risk of side effects, especially with prolonged use. The appearance of wrinkled skin on your glans could be a sign of skin thinning, which is a common side effect. It is essential to communicate openly with your healthcare provider about your symptoms and any concerns you have regarding your treatment. They can provide personalized advice and adjust your treatment plan as necessary to ensure both safety and efficacy. Always prioritize skin health and seek professional guidance when in doubt.
