What are the side effects of steroids?
Due to my keloids, I often receive steroid injections.
What are the potential side effects of these injections? When I asked doctors, they only mentioned that it could cause vascular prominence, but they seemed reluctant to provide more information, and one doctor even quickly dismissed my keloids, saying there was no treatment available, and rushed me out of the consultation room in less than a minute.
This left me feeling very disappointed and heartbroken.

Dr. Hong Ruiyou reply Pharmacology
Hello: The side effects of steroid administration can vary widely.
For individuals with renal insufficiency, edema may occur, and patients with a history of allergies should be cautious as it may trigger an anaphylactic shock reaction.
Abruptly discontinuing long-term treatment may lead to symptoms resembling adrenal insufficiency, including nausea, fatigue, anorexia, shortness of breath, hypotension, hypoglycemia, myalgia, fever, arthralgia, dizziness, desquamation, and fainting.
It is important to taper off the medication gradually.

When it comes to steroid injections for keloids, it's essential to understand both the therapeutic benefits and potential side effects associated with this treatment. Keloids are raised scars that occur due to an overproduction of collagen during the healing process. Steroid injections, particularly corticosteroids, are commonly used to reduce inflammation and flatten keloids.
Common Side Effects of Steroid Injections:
1. Skin Changes: One of the most common side effects is skin thinning or atrophy at the injection site. This can lead to a change in skin texture and appearance, making the skin more susceptible to bruising and injury.
2. Hypopigmentation: Steroid injections can cause lightening of the skin (hypopigmentation) in the treated area, which may be more noticeable in individuals with darker skin tones.
3. Vascular Changes: As you mentioned, one of the side effects can be the appearance of blood vessels (telangiectasia) under the skin. This occurs due to the weakening of the skin and blood vessels from the steroid.
4. Pain and Discomfort: Some patients may experience pain, swelling, or discomfort at the injection site. This can be temporary, but in some cases, it may persist.
5. Infection Risk: Although rare, there is a risk of infection at the injection site. Proper sterile techniques are crucial to minimize this risk.
6. Systemic Effects: While rare with localized injections, systemic side effects can occur, especially with high doses or frequent injections. These can include weight gain, mood changes, and increased blood sugar levels.
7. Recurrence of Keloids: While steroid injections can help flatten keloids, there is a possibility that they may recur after treatment. Some patients may require multiple sessions for optimal results.
Addressing Your Concerns:
It's understandable to feel frustrated and disappointed with your experiences with healthcare providers. Communication is crucial in any medical treatment, and it's important that you feel heard and informed about your treatment options. If you feel that your concerns about side effects were not adequately addressed, consider seeking a second opinion from a dermatologist or a specialist in scar management. They can provide you with a more comprehensive understanding of the treatment and any potential risks involved.
Additionally, there are alternative treatments for keloids, including silicone gel sheets, laser therapy, and surgical options. Each treatment has its own set of benefits and risks, and a thorough discussion with a knowledgeable healthcare provider can help you make an informed decision.
In conclusion, while steroid injections can be effective for managing keloids, they are not without potential side effects. It's essential to have open discussions with your healthcare provider about your treatment plan, including any concerns you may have regarding side effects and alternative options. Your health and well-being should always come first, and you deserve to receive the care and attention necessary to address your condition effectively.
