Can steroids cause dizziness?
Hello, Doctor.
I have had SLE for 15 years, and I was recently discharged from the hospital due to vasculitis.
I have been resting at home for ten days now and am currently taking eight corticosteroids.
I feel about 90% recovered, but I am troubled by insomnia and dizziness.
Is this dosage considered high? I remember that corticosteroids do not typically cause dizziness.
I don't feel dizzy when lying down, but when I sit up, even slight movements make me feel like I'm swaying.
I have had an MRI, and there are no issues with my brain.
I also have another question: what is the purpose of quinine? Is it possible to stop taking it? Thank you, Doctor, for taking the time to respond.

Dr. Gao Jiankai reply Internal Medicine
Hello Julia, thank you for your question.
Here is my response summarized as follows:
1.
The dosage of 8 steroid pills is not considered high, and it should not be used for a long duration; you can always consult your physician about this.
2.
Steroids are unlikely to cause dizziness; it is more probable that the dizziness is a result of sleep disturbances caused by steroid use.
During periods of higher steroid dosage, appropriate use of sleep aids may be necessary.
3.
Quinine is a good medication that can lower cholesterol, help reduce the dosage of steroids, and regulate immune function, among other benefits.
It currently has the least side effects among immune-modulating drugs.
However, some individuals may experience skin itching, hair loss, and darkening of the skin with long-term use, which may deter some female patients from using it.
Nevertheless, I believe it is an important medication, particularly suitable for SLE patients with fever, joint pain, rashes, and kidney inflammation.
Personally, I would try to persuade patients to use it.
Best regards from the Allergy, Immunology, and Rheumatology Department of Changhua Hospital, Ministry of Health and Welfare.

More Info
Steroids, particularly corticosteroids, are commonly used in the treatment of various autoimmune conditions, including systemic lupus erythematosus (SLE) and vasculitis. While they can be highly effective in managing inflammation and suppressing the immune system, they also come with a range of potential side effects. Your experience of dizziness, especially when transitioning from lying down to sitting, can indeed be associated with steroid use, although it is not one of the most commonly reported side effects.
Understanding Dizziness and Steroid Use
Dizziness can manifest in various forms, including lightheadedness, vertigo, or a sensation of imbalance. In the context of steroid therapy, several factors could contribute to your symptoms:
1. Fluid Retention: Corticosteroids can cause the body to retain sodium and water, leading to increased blood volume and potential fluctuations in blood pressure. This can result in feelings of dizziness, especially upon standing or sitting up quickly.
2. Blood Sugar Levels: Steroids can affect glucose metabolism, potentially leading to elevated blood sugar levels. This can cause symptoms such as dizziness, particularly if blood sugar levels fluctuate significantly.
3. Sleep Disturbances: As you mentioned experiencing insomnia, lack of restorative sleep can contribute to feelings of fatigue and dizziness. Sleep deprivation can affect cognitive function and balance, leading to a sensation of dizziness.
4. Medication Interactions: If you are taking other medications alongside steroids, interactions could also contribute to your symptoms. It’s essential to review all medications with your healthcare provider.
5. Underlying Conditions: Although your MRI showed no issues, it’s important to consider that other underlying conditions related to SLE or vasculitis could also contribute to dizziness.
Is Your Dosage High?
The dosage of steroids can vary significantly based on the condition being treated, the severity of the disease, and individual patient factors. Eight pills a day could represent a high dose, depending on the specific steroid and its strength. It’s crucial to discuss your current dosage with your healthcare provider, who can assess whether it is appropriate for your condition and whether any adjustments are necessary.
Quinine: Purpose and Considerations
Quinine is a medication primarily used to treat malaria and is sometimes prescribed for leg cramps. If you are taking quinine, it’s essential to understand its purpose in your treatment plan. If it was prescribed for a specific reason, such as managing symptoms related to your condition, it’s advisable to consult your physician before discontinuing it. Stopping any medication without professional guidance can lead to unintended consequences.
Recommendations
1. Consult Your Doctor: Given your symptoms of dizziness and the high steroid dosage, it’s essential to have a thorough discussion with your healthcare provider. They may recommend adjusting your steroid dosage or exploring other treatments to manage your symptoms.
2. Monitor Blood Pressure: Keep track of your blood pressure, especially when changing positions. If you notice significant fluctuations, inform your doctor.
3. Hydration and Diet: Ensure you are well-hydrated and maintain a balanced diet, as this can help mitigate some side effects of steroids.
4. Sleep Hygiene: Work on improving your sleep hygiene to combat insomnia. This may include establishing a regular sleep schedule, creating a restful environment, and avoiding stimulants before bedtime.
5. Follow-Up: Regular follow-up appointments are crucial to monitor your condition and adjust treatments as necessary.
In conclusion, while dizziness is not a classic side effect of steroids, it can occur due to various factors related to their use. It’s vital to communicate openly with your healthcare provider about your symptoms and concerns to ensure you receive the best possible care.
