Four days after cataract surgery, there is still blood in the eye and vision remains unclear?
Hello, my father underwent micro-incision cataract surgery four days ago, and he still cannot see clearly.
He reports seeing a bright white light, but everything appears blurry.
An ophthalmic examination showed that his vision is still only 0.15, and there is still blood in his eye.
My father is very worried about whether he will be unable to see clearly in the future, so I would like to ask if it is abnormal for there to still be blood in his eye.
The doctor did not mention anything during the follow-up visit, but my father remains concerned.
Thank you.

Dr. Cai Wenyuan reply Ophthalmology
Hello: Is it abnormal for the eyes to still have blood? If it's just blood, it should become clearer after it disperses, but regular follow-up examinations are recommended.
Wishing you safety and health.
Sincerely, Dr.

Post-cataract surgery complications can be concerning, especially when symptoms such as blurry vision and the presence of blood in the eye occur. Your father's situation, where he is experiencing persistent blurry vision and has noted blood in his eye four days after the surgery, warrants careful consideration and monitoring.
Firstly, it is important to understand that some degree of blurry vision can be expected immediately following cataract surgery. The eye is healing, and the visual system is adjusting to the new intraocular lens (IOL) that has been implanted. However, the presence of blood in the eye, known as a hyphema, is not typical and can indicate a complication. Hyphema can occur due to various reasons, including trauma during surgery, bleeding from the iris, or other underlying conditions.
In your father's case, the fact that he is seeing bright lights but objects appear blurry suggests that while the IOL may be functioning, there could be other factors affecting his vision. The blood in the eye could be contributing to the blurriness, as it can obstruct vision and cause light to scatter, leading to visual disturbances.
Typically, after cataract surgery, the blood should gradually dissipate, and vision should improve as the eye heals. If the blood is not reabsorbing or if the vision does not improve, it may indicate a need for further evaluation. It is crucial to follow up with the ophthalmologist to monitor the situation closely. They may perform a thorough examination, including checking the intraocular pressure and assessing the retina and other structures within the eye to rule out any serious complications such as retinal detachment or significant bleeding.
Regarding your father's concern about whether he will have lasting vision issues, it is essential to communicate with the eye care provider. While many patients experience excellent outcomes after cataract surgery, complications can arise, and the prognosis can vary based on the individual case. The ophthalmologist can provide the best insight into your father's specific situation, including the expected timeline for recovery and any necessary interventions.
In summary, while some blurry vision can be normal after cataract surgery, the presence of blood in the eye is concerning and should be evaluated by an eye care professional. Encourage your father to keep his follow-up appointments and to communicate any changes in his symptoms. If he experiences increased pain, worsening vision, or any other alarming symptoms, he should seek immediate medical attention. Early intervention can be crucial in addressing any complications and ensuring the best possible outcome for his vision.
