the Itching: Benzyl Benzoate for Scabies Prevention - Dermatology

Preventive application of scabies medication?

Hello, Director Wu.
My brother was diagnosed with scabies by a dermatologist the day before yesterday.
(However, I do not sleep in the same room as him, and I have always washed my clothes by hand.) The doctor said that if one person has scabies, the whole family needs to disinfect to prevent ping-pong transmission.
Therefore, I am currently applying Benzyl benzoate solution as a preventive measure.
After showering, I apply the BB solution all over my body (from the neck down, including behind the ears and between the fingers).
Today is the second day I have been applying the solution, and I experience a stinging sensation each time I apply it (which alleviates after 1-2 hours).
I do not have any itching on my skin, and I have not noticed any rashes.
I would like to ask you, Director Wu, is the stinging sensation due to the presence of scabies mites on my skin, or is it because the BB solution itself is irritating, causing the stinging sensation?

Ms. Hsu, 20~29 year old female. Ask Date: 2017/08/02

Dr. Wu Yuxin reply Dermatology

The irritant nature of the medication itself.

Benzyl benzoate is a topical medication commonly used to treat scabies, a skin infestation caused by the Sarcoptes scabiei mite. It is effective in killing the mites and their eggs, thereby alleviating the symptoms associated with scabies. However, it is important to understand the potential side effects and the mechanism behind the sensations you are experiencing while using this medication.

The tingling or stinging sensation you feel after applying benzyl benzoate is likely due to the medication itself rather than an indication that you have scabies. Benzyl benzoate can be irritating to the skin, especially if it is applied to areas that are already inflamed or sensitive. This irritation can manifest as a temporary stinging or burning sensation, which typically subsides after a short period, as you have noted. It is essential to follow the application instructions provided by your healthcare provider and to ensure that the skin is clean and dry before applying the solution.

In terms of scabies prevention, your doctor’s advice to treat all family members is crucial. Scabies is highly contagious and can be transmitted through close physical contact or by sharing clothing, bedding, or towels. Even if you are not showing symptoms, it is wise to take preventive measures, as the mites can be present on the skin without causing immediate itching or rash. The use of benzyl benzoate as a preventive measure is appropriate, especially in a household where one member has been diagnosed with scabies.

To further enhance your understanding, it is important to note that the itching associated with scabies is primarily caused by an allergic reaction to the mites, their eggs, and their waste products. This reaction can take several weeks to develop after initial exposure, which is why individuals may not experience symptoms immediately. In your case, since you have not developed any itching or rashes, it is possible that you have not been infested, or your skin is reacting differently.

When using benzyl benzoate, it is also essential to consider the following recommendations:
1. Application Technique: Apply the solution thoroughly to all areas of the body from the neck down, including the spaces between fingers and toes, behind the ears, and under the nails. This ensures that any potential mites are effectively treated.

2. Clothing and Bedding: Wash all clothing, bedding, and towels in hot water and dry them on a hot cycle to kill any mites that may be present. Items that cannot be washed should be sealed in plastic bags for at least 72 hours.

3. Follow-Up: If symptoms persist or worsen, or if you develop a rash or severe irritation, consult your healthcare provider. They may recommend an alternative treatment or further evaluation.

4. Avoiding Irritation: If the stinging sensation becomes uncomfortable, you might want to discuss with your doctor whether to continue using benzyl benzoate or consider alternative treatments that may be less irritating.

In summary, while the stinging sensation you are experiencing is likely due to the benzyl benzoate itself, it is essential to continue monitoring your skin for any changes. Following your doctor's recommendations for treating and preventing scabies is crucial for the health and safety of you and your family. If you have any concerns about the medication or its side effects, do not hesitate to reach out to your healthcare provider for guidance.
