Glenital eczema, could you please provide more details?

Hello doctor, I would like to ask if the surface of the glans is no longer rough and feels smooth like before, does that mean the eczema has healed? (I stopped the medication 4 days ago and have been applying papaya ointment during these 4 days.) There are still a few pinhead-sized red spots on the glans that appeared after applying the eczema medication.
When exposed to heat, they resemble whiteheads but are not painful or itchy (I don't dare to pick at them).
What could this be? The red spots disappear when I have an erection (which is strange)! Currently, I am still applying Australian papaya ointment for maintenance; is that appropriate? It has been over a month without masturbation, and after ejaculation the day before yesterday, there were no abnormalities.
However, today after ejaculation (from masturbation), I noticed no blood when wiping, but after waiting 10 minutes to urinate, there was a streak of blood.
After that, there was no blood in my urine.
Could this be a ruptured blood vessel?

An, 30~39 year old female. Ask Date: 2017/08/11

Dr. Du Shixi reply Urology

Hello: Normal skin is fine, but if there is redness or red spots, there may still be an issue.
It is not recommended to use ointments.
If there is blood in the urine after masturbation, a urology examination is advised (as there could be many reasons).
Wishing you good health.

More Info

Understanding glans eczema, also known as balanitis, involves recognizing its symptoms, treatment options, and potential concerns. Based on your description, it seems you are experiencing some residual symptoms after a treatment for eczema on the glans (the head of the penis).

Symptoms of Glans Eczema
Glans eczema can manifest in various ways, including:
1. Redness and Inflammation: The glans may appear red and inflamed, which is often accompanied by discomfort.

2. Dryness and Flaking: The skin may become dry, leading to flaking or peeling.

3. Itching or Burning Sensation: While you mentioned that you do not feel itching or burning, these are common symptoms associated with eczema.

4. Red Dots or Papules: The appearance of small red dots, like the ones you described, can occur. These may be related to the eczema or could indicate another condition, such as folliculitis or a reaction to irritants.

Treatment Options
Treatment for glans eczema typically involves:
1. Topical Steroids: These are often prescribed to reduce inflammation and alleviate symptoms. It’s crucial to follow your doctor’s instructions regarding the duration of use to avoid potential side effects.

2. Moisturizers: Using emollients or moisturizers, like the Australian papaya cream you mentioned, can help maintain skin hydration and barrier function.

3. Avoiding Irritants: Identifying and avoiding potential irritants, such as soaps, detergents, or certain fabrics, can help prevent flare-ups.

4. Antibiotics: If there is a secondary bacterial infection, topical or oral antibiotics may be necessary.

Concerns Regarding Your Symptoms
1. Red Dots: The small red dots you are observing could be a result of the eczema or a separate issue. If they are not painful or itchy, they may not be a cause for concern, but monitoring their appearance is essential. If they persist or worsen, consulting a healthcare provider is advisable.

2. Ejaculation and Blood in Urine: The presence of blood in urine, even if it is just a trace, should not be ignored. It could be due to several reasons, including:
- Irritation or Trauma: Sometimes, vigorous activity or irritation can lead to minor bleeding.

- Infection: Urinary tract infections (UTIs) can cause blood in urine and should be evaluated.

- Vascular Issues: As you suggested, a ruptured blood vessel could be a possibility, especially if the bleeding is minimal and resolves quickly.

Recommendations
1. Follow-Up: Since you have experienced blood in your urine, it is crucial to follow up with a healthcare provider. They may recommend a urinalysis or other tests to rule out infections or other underlying conditions.

2. Continue Moisturizing: Using the papaya cream is generally safe, but ensure it does not irritate the area further. If you notice any adverse reactions, discontinue use and consult your doctor.

3. Monitor Symptoms: Keep an eye on any changes in your symptoms, including the red dots and any discomfort. Documenting these changes can be helpful for your healthcare provider.

4. Avoid Irritation: Be gentle with the area, especially during activities like masturbation, to prevent further irritation or trauma.

In conclusion, while it appears that your eczema may be improving, the presence of red dots and the episode of blood in your urine warrant further evaluation. Regular follow-ups with your healthcare provider will ensure that any potential issues are addressed promptly, allowing for a more effective management of your symptoms.
