Phimosis issue

I would like to inform the doctor that my family says I had a circumcision when I was young, and since then, I have not been able to retract the foreskin to expose the glans.
Even during erections, I have been experiencing phimosis.
Two days ago, I managed to retract the foreskin, but I found that it got stuck behind the glans, causing swelling and pain.
The next morning, I reverted the foreskin back to its phimosis state.
After researching online, I believe my condition may be paraphimosis.
I would like to ask the doctor if I need to undergo surgery to improve the situation and prevent pain when exposing the glans, or if there are any methods I can use to gradually resolve the paraphimosis on my own.
Thank you, doctor.

Jie, 10~19 year old female. Ask Date: 2017/09/04

Dr. Du Shixi reply Urology

Hello: There is just a small area of tightness, not a case of phimosis.
However, you should seek medical examination to determine if surgery is necessary.
Wishing you good health.

More Info

Phimosis is a condition characterized by the inability to retract the foreskin over the glans (the head of the penis). It can be a source of discomfort and complications, particularly when it leads to conditions like paraphimosis, where the retracted foreskin cannot return to its original position, causing swelling and pain. Understanding the causes, symptoms, and treatment options for phimosis is essential for managing this condition effectively.

Causes of Phimosis
Phimosis can be classified into two types: physiological and pathological.
1. Physiological Phimosis: This is common in infants and young boys, where the foreskin is naturally non-retractable due to the adhesion between the glans and the foreskin. This condition usually resolves on its own as the child grows.

2. Pathological Phimosis: This occurs due to scarring, inflammation, or infection, which can result from conditions such as balanitis (inflammation of the glans), recurrent infections, or trauma. In adults, it may also be associated with conditions like diabetes, which can lead to skin changes and infections.

Symptoms of Phimosis
The symptoms of phimosis can vary but may include:
- Inability to retract the foreskin
- Pain or discomfort during erections or sexual activity
- Swelling or redness of the glans
- Difficulty with hygiene, leading to recurrent infections
- In severe cases, paraphimosis, where the retracted foreskin cannot return to its original position, leading to swelling and pain.

Treatment Options
The treatment for phimosis depends on the severity of the condition and the symptoms experienced. Here are some options:
1. Conservative Management: In cases where phimosis is not causing significant problems, conservative management may be sufficient. This can include:
- Gentle Stretching Exercises: Gradually stretching the foreskin can help in some cases. This should be done carefully to avoid pain or injury.

- Topical Steroid Creams: Applying a topical steroid cream prescribed by a healthcare provider can help reduce inflammation and promote the retraction of the foreskin.

2. Surgical Options: If conservative measures fail or if there are complications such as recurrent infections or paraphimosis, surgical intervention may be necessary. Options include:
- Circumcision: This is the complete removal of the foreskin and is a definitive treatment for phimosis. It is often recommended for recurrent issues.

- Preputioplasty: This is a less invasive procedure that involves widening the opening of the foreskin without removing it. This can be an option for those who wish to retain their foreskin.

Your Situation
Based on your description, it seems you are experiencing symptoms consistent with paraphimosis after attempting to retract the foreskin. This condition can lead to significant pain and swelling, and it is essential to address it promptly.
While some individuals may manage mild cases of phimosis with stretching and topical treatments, your experience of pain and swelling suggests that you should seek medical attention. A healthcare provider can assess the severity of your condition and recommend the most appropriate treatment.
In cases of paraphimosis, immediate medical intervention is often necessary to relieve the swelling and pain. This may involve manual reduction of the foreskin or, in some cases, surgical intervention if the condition does not resolve.

Conclusion
In summary, phimosis can lead to complications like paraphimosis, which requires careful management. While some individuals may find relief through conservative measures, others may need surgical intervention. It is crucial to consult with a healthcare provider to determine the best course of action for your specific situation. Do not hesitate to seek medical advice, especially given the pain and discomfort you are experiencing.
