Nasopharyngeal carcinoma and lymphatic issues?

Hello, Doctor.

1.
If nasopharyngeal carcinoma leads to enlarged cervical lymph nodes, will the lumps increase in size over time? Typically, how large can they become?
2.
Recently, I noticed a few enlarged lymph nodes on the left side of my neck, approximately 1 cm in size, with one near the mandible that is elongated (about the length of a finger joint).
I have seen several general practitioners, and they all said there was no significant issue.
However, I am very concerned that there might be a problem.

3.
Recently, I have been experiencing a slight itch in my throat, with a sensation of wanting to cough, but I am not actually coughing.
I only cough a few times when I press on the area below my neck near the trachea.
Is this inflammation? Thank you, Doctor!

Dr. Zheng Jueyi reply Otolaryngology

Hello: 1.
Nasopharyngeal carcinoma can lead to cervical lymphadenopathy, with the masses typically increasing in size over time, generally exceeding one centimeter and continuing to grow.
2.
If concerned, regular follow-up appointments for examination are recommended.
3.
Based on the symptoms, it does not appear to be inflammatory.
Wishing you peace and health.
Sincerely, Dr.
Cheng Jue-Yi, Head of the ENT Department, Taoyuan Hospital, Ministry of Health and Welfare.

Hello! Thank you for your questions regarding nasopharyngeal cancer and lymph node concerns. I understand that you are feeling anxious about your symptoms and the condition of your lymph nodes. Let’s address your questions one by one.

1. Lymph Node Enlargement and Nasopharyngeal Cancer: If nasopharyngeal cancer leads to cervical lymphadenopathy (enlarged lymph nodes in the neck), the lymph nodes can indeed increase in size over time. The rate of enlargement can vary significantly among individuals. Some lymph nodes may remain stable, while others can grow larger, sometimes reaching sizes of several centimeters. Typically, lymph nodes that are associated with malignancies may be larger than 1 cm, but this is not a strict rule. The presence of other symptoms, such as weight loss, night sweats, or persistent fever, may also indicate a more aggressive process.

2. Concerns About Your Enlarged Lymph Nodes: You mentioned that you have noticed a few enlarged lymph nodes on the left side of your neck, with one being about 1 cm in size and elongated in shape. It is reassuring that multiple doctors have examined these nodes and deemed them not concerning. However, it is completely normal to feel anxious about changes in your body, especially when it comes to potential malignancies. The characteristics of lymph nodes, such as their size, shape, and mobility, can provide clues about their nature. Generally, benign lymph nodes tend to be more mobile and have a regular shape, while malignant nodes may be fixed, irregular, or hard. If you continue to feel uneasy, consider seeking a second opinion or requesting imaging studies like an ultrasound or CT scan for further evaluation.

3. Throat Symptoms and Inflammation: The itchy sensation in your throat and the urge to cough could be indicative of several conditions, including allergies, a viral infection, or even mild inflammation. If you only cough when pressing on your trachea, it may suggest that there is some irritation in that area, but it does not necessarily indicate a serious problem. If the symptoms persist or worsen, it would be wise to consult an ENT specialist who can perform a thorough examination of your throat and neck.

In summary, while lymph node enlargement can be concerning, especially in the context of nasopharyngeal cancer, many factors influence their behavior. The fact that multiple healthcare providers have assessed your lymph nodes and found them unremarkable is a positive sign. However, if you have ongoing concerns or if your symptoms change, do not hesitate to seek further evaluation. It's essential to listen to your body and advocate for your health.

If you have any more questions or need further clarification, feel free to ask. Take care!
