Ear Noises: Causes and Remedies for Ear Symptoms - Otolaryngology

Tinnitus?

When swallowing or yawning, there is a popping sound in the left ear.
When lying down with my head tilted to the left, there is no sound in the left ear; however, when lying on the right side, the left ear produces a sound that resembles a pulse, synchronized with my heartbeat.
Occasionally, there are sounds similar to earwax, but after some time, I realize it is likely coming from within the ear.
The sound is not loud and does not affect my hearing.
This issue has only started recently and has been ongoing for about half a month; I did not have these problems before.
What methods can be used to improve this condition?

Rongrong, 20~29 year old female. Ask Date: 2017/09/14

Dr. Li Jiaxuan reply Otolaryngology

It is generally recommended to conduct tympanometry and endoscopic examination for the symptoms mentioned above.
If any issues are found, imaging studies may be performed.
Given your age, it could be due to allergic adenoid hypertrophy, but nasopharyngeal tumors cannot be ruled out.
It is advised to visit an otolaryngology clinic.
Wishing you good health.

Understanding ear noises, especially when they are accompanied by sensations like popping or pulsing sounds, can be quite perplexing. Based on your description, it seems you are experiencing a variety of auditory phenomena in your left ear, particularly when swallowing, yawning, or changing positions. These sounds can be attributed to several potential causes, which I will outline below, along with possible remedies.

Potential Causes of Ear Noises
1. Eustachian Tube Dysfunction: The Eustachian tube connects the middle ear to the back of the nose and helps equalize ear pressure. When this tube is not functioning properly, it can lead to sensations of fullness, popping sounds, or even a feeling of pressure in the ear. This dysfunction can be triggered by allergies, colds, or sinus infections.

2. Earwax Buildup: While you mentioned that the sounds sometimes resemble earwax, it’s important to note that a buildup of earwax can cause various auditory sensations, including muffled hearing or noises that seem to resonate within the ear. If the earwax is impacted, it can also affect the Eustachian tube's ability to function properly.

3. Muscle Spasms: The muscles in the ear can sometimes spasm, leading to sounds that might resemble clicking or popping. This is often benign and can occur due to stress or fatigue.

4. Pulsatile Tinnitus: The pulsing sound you hear in sync with your heartbeat could be a form of tinnitus known as pulsatile tinnitus. This condition can be caused by changes in blood flow or vascular issues near the ear.

5. Changes in Pressure: Lying down or changing head positions can alter the pressure in the ear, leading to sounds that may not be present when you are upright. This is particularly common when the Eustachian tube is not functioning optimally.

Remedies and Recommendations
1. Yawning and Swallowing: Continue to yawn or swallow frequently, as these actions can help open the Eustachian tube and equalize pressure in the ear. This may alleviate some of the popping sounds.

2. Nasal Decongestants: If you suspect that nasal congestion is contributing to your symptoms, over-the-counter decongestants or saline nasal sprays may help reduce swelling in the nasal passages and improve Eustachian tube function.

3. Earwax Removal: If you suspect earwax buildup, consider consulting a healthcare professional for safe removal. Avoid using cotton swabs, as they can push wax further into the ear canal.

4. Warm Compress: Applying a warm compress to the affected ear may help relax the muscles and alleviate discomfort.

5. Stay Hydrated: Keeping well-hydrated can help thin mucus and promote better Eustachian tube function.

6. Avoid Loud Noises: Protect your ears from loud environments, as excessive noise can exacerbate tinnitus and other ear-related issues.

7. Consult an ENT Specialist: If your symptoms persist or worsen, it’s advisable to see an Ear, Nose, and Throat (ENT) specialist. They can perform a thorough examination, including hearing tests and possibly imaging studies, to rule out any underlying conditions.

Conclusion
While the sounds you are experiencing in your left ear may not currently affect your hearing, they can be bothersome. Understanding the potential causes and implementing some of the suggested remedies may provide relief. However, if symptoms persist, seeking professional medical advice is crucial to ensure there are no underlying issues that require treatment. Your ear health is important, especially if you are exposed to environments that may affect your hearing, such as flying or attending loud events.
