The presence of black, fibrous particles in semen?
Hello Dr.
Du,
I am currently 26 years old.
I recently discovered black, fibrous material in my semen, which I experienced a long time ago but had not seen for several months.
After researching, I am still uncertain whether this is due to seminal vesiculitis or another condition.
My previous lifestyle habits included getting a full 7 hours of sleep daily (no late nights) and masturbating every 3-4 days (with normal semen each time), lasting about 20 minutes.
In the past two weeks, I have changed my routine to also include waking up at 6:00 AM to run 2000 meters or go to the gym (alternating), and running 3000 meters in the evening between 8:30 and 9:30 PM.
My masturbation frequency and duration remain unchanged, and I have not experienced any pain.
I do not smoke or have a history of alcohol abuse.
I would like to ask you, Dr.
Du:
1.
What could be the possible causes of this black, fibrous material?
2.
If this black, fibrous material disappears again, do I still need to see a urologist for an examination?
Thank you for taking the time to clarify these concerns.

Hello: It is mostly just a difference in the color of the semen, but it is necessary to examine the semen.

When it comes to the presence of black particles in semen, it can understandably cause concern. The appearance of these particles can be attributed to several factors, and understanding the potential causes is crucial for determining whether medical attention is necessary.
1. Possible Causes: The black particles you are observing in your semen could be due to a few different reasons. One common cause is the presence of blood, which can sometimes appear dark or black when mixed with semen. This could be due to a variety of conditions, including but not limited to:
- Infection: Conditions such as prostatitis (inflammation of the prostate) or seminal vesiculitis (inflammation of the seminal vesicles) can lead to the presence of blood in the semen, which may appear as dark particles.
- Trauma: Any trauma to the genital area can also lead to bleeding, which may manifest as dark particles in the semen.
- Varicocele: This is a condition characterized by enlarged veins within the scrotum, which can sometimes lead to changes in semen appearance.
- Benign Prostatic Hyperplasia (BPH): An enlargement of the prostate can also lead to changes in semen consistency and appearance.
- Other Medical Conditions: Less commonly, conditions like tumors or other abnormalities in the reproductive system could lead to similar symptoms.
2. When to Seek Help: If the black particles in your semen persist, recur, or are accompanied by other symptoms such as pain, discomfort, changes in urination, or any other unusual symptoms, it is advisable to seek medical attention. A urologist can perform a thorough examination, which may include a physical exam, urine tests, and possibly imaging studies or semen analysis to determine the underlying cause.
3. If the Particles Disappear: If the black particles have disappeared and you are not experiencing any other symptoms, it may not be immediately necessary to seek medical attention. However, it is essential to remain vigilant. If you notice any recurrence or new symptoms, it would be wise to consult a healthcare professional.
4. Lifestyle Factors: Your lifestyle habits, such as regular exercise and adequate sleep, are generally positive and can contribute to overall health. However, if you have made significant changes to your routine, it may be worth considering how these changes could impact your body. Stress, diet, and hydration can also play roles in reproductive health.
In conclusion, while the presence of black particles in semen can be alarming, it is essential to consider the context and any accompanying symptoms. If the issue persists or if you have any concerns, consulting a urologist for a professional evaluation is the best course of action. They can provide a more accurate diagnosis and appropriate treatment options based on your specific situation. Remember, proactive health management is key to maintaining reproductive health.
