The skin of the penis is prone to tearing?

Hello, Doctor: I have undergone circumcision, and after having intercourse with my wife, I experience skin tearing and bleeding at the junction of the glans and the shaft of the penis.
I apply lubricant beforehand and supplement it during the process if it feels too dry.
Sometimes I feel pain during intercourse, and afterward, I notice bleeding.
Applying ointment helps it heal in two days, but before the next time, I apply lubricant under the glans and also some on the outside of the condom, yet I still experience skin tearing and bleeding.
Both my wife and I have only had each other as sexual partners, and I didn't have any issues before when I didn't use lubricant.
Additionally, I have noticed red spots on the glans that resemble eczema over the past year; they are not itchy or painful.
What could this issue be? Thank you!

Hello: It is likely that an infection is causing the skin to become easily damaged due to friction.
You should consult a urologist at the hospital.
If the parents have diabetes, it is also advisable to check the blood sugar levels.
Wishing you good health.

When it comes to penile skin issues, particularly pain and bleeding during intercourse, there are several factors to consider. Your experience of bleeding and pain at the junction of the glans (the head of the penis) and the shaft, despite using lubricant, suggests that there may be underlying issues that need to be addressed.

First, it’s important to note that the skin in this area is sensitive and can be prone to irritation. Even with the use of lubricants, friction during intercourse can lead to micro-tears in the skin, especially if the skin is dry or if there is an underlying condition affecting the skin's integrity. The fact that you have previously undergone circumcision may also play a role, as the skin may be more sensitive or less elastic than before.

The bleeding you are experiencing could be due to several reasons:
1. Friction and Irritation: Even with lubrication, if there is excessive friction during intercourse, it can lead to skin irritation and micro-tears, resulting in bleeding. It’s crucial to ensure that both partners are adequately aroused and that sufficient lubrication is used throughout the entire intercourse.

2. Skin Conditions: The red spots you mentioned on the glans that resemble eczema but are not itchy or painful could indicate a dermatological condition such as contact dermatitis, psoriasis, or even a fungal infection. These conditions can cause changes in the skin that may make it more susceptible to injury.

3. Infections: Although you mentioned that both you and your partner are monogamous, it’s still possible to have infections that are not sexually transmitted but can cause irritation or inflammation in the genital area. Conditions like balanitis (inflammation of the glans) can also lead to similar symptoms.

4. Allergic Reactions: Sometimes, the lubricants or condoms used can cause allergic reactions or sensitivities that lead to irritation and subsequent bleeding. It may be worth trying different types of lubricants (such as water-based or silicone-based) to see if that alleviates the problem.

Given that you have been experiencing these symptoms consistently, it is advisable to consult a healthcare professional, preferably a urologist or dermatologist specializing in genital skin issues. They can perform a thorough examination, possibly including a biopsy if necessary, to determine the exact cause of your symptoms.

In the meantime, here are some recommendations:
- Use Adequate Lubrication: Ensure that you are using a sufficient amount of lubricant during intercourse. If you find that a particular lubricant is causing irritation, consider switching to a hypoallergenic option.

- Gentle Techniques: Consider adopting gentler techniques during intercourse to minimize friction. Communication with your partner about comfort levels is essential.

- Avoid Irritants: If you suspect that certain products (like soaps or lubricants) may be causing irritation, try to eliminate them from your routine.

- Moisturize: After intercourse, applying a gentle, fragrance-free moisturizer may help soothe the skin and prevent dryness.

- Monitor Symptoms: Keep track of any changes in the appearance of the skin or any new symptoms that arise, as this information will be helpful for your healthcare provider.

In conclusion, while it’s possible that the bleeding and pain you are experiencing are related to friction and skin sensitivity, it’s essential to rule out any underlying conditions. A healthcare professional can provide a proper diagnosis and treatment plan tailored to your specific needs.
