Should circumcision be performed?
Hello doctor, this issue has been troubling me for a long time.
My glans is usually covered by the foreskin, and during intercourse with my girlfriend, the glans can come out smoothly.
I can also retract the foreskin normally, but if I keep the foreskin retracted for too long without covering the glans, the blood vessels in the foreskin cannot circulate properly overnight, leading to swelling.
Whenever this happens, I quickly cover the glans with the foreskin, and then the swollen foreskin gradually reduces in size.
I would like to ask if there are any methods to improve this situation so that the foreskin can remain retracted behind the glans without causing vascular congestion overnight.

It sounds like there may still be a tight ring of foreskin, which typically isn't an issue during normal conditions, but can constrict the penis during an erection, leading to impaired blood flow.
However, whether or not you need a circumcision can only be determined through an in-person evaluation by a physician.

More Info
Circumcision is a surgical procedure that involves the removal of the foreskin from the penis. It is a topic that often raises questions regarding its necessity, benefits, and potential health implications. In your case, it seems you are experiencing issues with your foreskin that could be indicative of a condition known as phimosis, where the foreskin cannot be easily retracted over the glans (the head of the penis). This can lead to discomfort, swelling, and other complications if not addressed properly.
Health Implications of Circumcision
1. Hygiene: One of the primary benefits of circumcision is improved hygiene. The removal of the foreskin makes it easier to clean the glans, reducing the risk of infections such as balanitis (inflammation of the glans) and other conditions associated with poor hygiene.
2. Reduced Risk of Infections: Studies have shown that circumcision can reduce the risk of urinary tract infections (UTIs) in infants and may lower the risk of sexually transmitted infections (STIs) in adults, including HIV. This is particularly relevant for sexually active individuals.
3. Phimosis and Paraphimosis: As you described, if the foreskin cannot be retracted easily, it can lead to complications such as phimosis or paraphimosis (where the retracted foreskin cannot return to its original position). These conditions can cause pain and swelling and may require medical intervention.
4. Sexual Function: Some men report enhanced sexual pleasure after circumcision, while others may experience a decrease in sensitivity. This varies from person to person and is often subjective.
5. Cultural and Religious Considerations: In some cultures and religions, circumcision is a common practice for newborns or young boys. This can influence personal decisions regarding the procedure.
Alternatives to Circumcision
If you are hesitant about undergoing circumcision, there are alternative approaches to manage your current situation:
1. Topical Steroids: In some cases, applying a topical steroid cream can help to loosen the foreskin and make it easier to retract. This should be done under the guidance of a healthcare professional.
2. Gentle Stretching Exercises: Gradually and gently stretching the foreskin can sometimes help to alleviate phimosis. This should be done carefully to avoid pain or injury.
3. Regular Hygiene Practices: Maintaining good hygiene by cleaning the area regularly can help prevent infections and other complications.
4. Consultation with a Urologist: It is advisable to consult with a urologist who can provide a thorough examination and discuss the best options for your specific situation. They can help determine whether circumcision is necessary or if other treatments may be more appropriate.
Conclusion
In summary, circumcision can offer several health benefits, particularly in terms of hygiene and reducing the risk of certain infections. However, it is essential to weigh these benefits against your personal circumstances and preferences. If you are experiencing recurrent issues with your foreskin, it is crucial to seek medical advice to explore your options. A healthcare professional can provide tailored recommendations based on your health status and lifestyle, ensuring that you make an informed decision that aligns with your needs.
