Post-Surgery Concerns: Unexplained Anal Discharge After Hemorrhoidectomy - Gastroenterology and Hepatology

Colon issues

Hello Dr.
Chen, I had a hemorrhoidectomy (internal hemorrhoids) two months ago.
The wound has healed, I don't feel any pain, and I don't feel like I've had surgery anymore.
However, I have been experiencing involuntary leakage of a very light yellow or brownish fluid from my anus, which sometimes has a fecal odor or a fishy smell.
After having a bowel movement, the leakage is more pronounced, but it is less frequent at other times.
Why is this happening? The doctor at my follow-up appointment said that both the wound and the anus look normal.
My bowel movements are normal, with yellow or brownish long-shaped stools.
I don't have any abdominal pain or diarrhea, and my stools are soft and moist.
My appetite is good, and my diet is normal.
Is there something wrong with my intestines? I have lost 5 kg since the surgery.
What should I do? Should I see a gastroenterologist or return to the colorectal specialist? Thank you for your help, doctor.

After undergoing a hemorrhoidectomy, it is not uncommon for patients to experience some changes in bowel habits and anal discharge. In your case, the presence of a light yellow or brownish discharge, especially after bowel movements, can be concerning, but it is essential to understand the potential causes and when to seek further medical evaluation.

Firstly, it is important to note that after surgery, the anal area may take some time to heal completely. Even if the surgical site appears to be healed and you are not experiencing pain, it is possible for some residual discharge to occur as the body continues to adjust postoperatively. This discharge can be a result of several factors:
1. Residual Mucus Production: The intestines and rectum naturally produce mucus to facilitate bowel movements. After surgery, there may be an increase in mucus production as the body adapts to the changes in the anal area.

2. Changes in Bowel Function: Following a hemorrhoidectomy, some patients may experience alterations in bowel habits, including changes in stool consistency and frequency. If your stools are soft and moist, this could contribute to increased mucus or discharge.

3. Infection or Inflammation: Although your doctor has indicated that the surgical site appears normal, it is still possible for a low-grade infection or inflammation to develop, which could lead to abnormal discharge. Signs of infection may include increased redness, swelling, or a foul odor.

4. Fistula Formation: In some cases, a fistula (an abnormal connection between the rectum and the skin) can develop after surgery, leading to persistent discharge. This is less common but should be considered if the discharge continues or worsens.

Given that you have lost weight (5 kg) since the surgery, it is crucial to monitor your overall health. Weight loss can sometimes indicate underlying issues, especially if it is unintentional. While your appetite and bowel movements seem normal, persistent discharge and weight loss warrant further investigation.

Recommendations:
1. Follow-Up with a Specialist: Since you are experiencing ongoing discharge, it would be prudent to return to a colorectal surgeon or a gastroenterologist for a thorough evaluation. They can perform a physical examination and possibly imaging studies to rule out any complications such as a fistula or infection.

2. Monitor Symptoms: Keep track of any additional symptoms that may arise, such as fever, increased pain, or changes in bowel habits. This information can be helpful for your healthcare provider.

3. Diet and Hydration: Ensure you are maintaining a balanced diet and staying hydrated. This can help with bowel regularity and overall health.

4. Avoid Self-Diagnosis: While it is natural to worry about potential issues, avoid jumping to conclusions about what might be wrong. A healthcare professional can provide a more accurate diagnosis based on clinical findings.

In summary, while some discharge after a hemorrhoidectomy can be normal, persistent or unusual symptoms should not be ignored. It is advisable to seek further evaluation to ensure that there are no underlying complications. Your health and well-being are paramount, and addressing these concerns with a specialist will provide you with the best course of action.
