Will taking Bicalutamide affect muscle growth? Does it suppress testosterone secretion?

Dr. Huang Shenggang reply Pharmacology

Hello: Regarding the side effects of this medication, as stated in the package insert, 1.
This drug has good tolerability.
In a controlled clinical study involving 543 patients over a period of 12 months, where patients were treated daily with 5mg of PROSCAR, 7 patients discontinued the medication due to side effects related to PROSCAR.
The adverse reactions related to sexual function were the most commonly reported side effects; however, among the aforementioned 7 patients, only 1 stopped PROSCAR treatment due to this specific side effect.
2.
In these studies, researchers classified adverse reactions as possibly, probably, or definitely drug-related, with an incidence greater than 1.0% and higher than that of the placebo group.
The results were: impotence (3.7%), decreased libido (3.3%), and reduced semen volume upon ejaculation (8%).
Thank you for your inquiry, and I hope this information is helpful to you.

Proscar, known generically as finasteride, is primarily used to treat conditions like benign prostatic hyperplasia (BPH) and male pattern baldness by inhibiting the enzyme 5-alpha reductase. This enzyme is responsible for converting testosterone into dihydrotestosterone (DHT), a more potent androgen that plays a significant role in hair loss and prostate growth. Your question about whether Proscar affects muscle growth and testosterone levels is quite relevant, especially for individuals engaged in bodybuilding or fitness.

To address your concerns, it is important to understand the relationship between testosterone, DHT, and muscle growth. Testosterone is a key hormone that contributes to muscle hypertrophy (growth) and strength. While DHT is derived from testosterone, it is not directly responsible for muscle growth. Instead, testosterone itself is the primary androgen that promotes muscle protein synthesis and overall muscle development.

Research indicates that finasteride can lower DHT levels significantly, which may lead to some hormonal changes. However, the impact on testosterone levels is more nuanced. In many cases, the use of finasteride does not significantly lower testosterone levels; instead, it may cause a slight increase in circulating testosterone due to the reduced conversion to DHT. This means that while DHT levels drop, testosterone levels may remain stable or even increase slightly.
However, the potential side effects of finasteride, particularly regarding sexual function, have been well-documented. Some users report decreased libido, erectile dysfunction, and reduced semen volume. These side effects can indirectly affect your workout motivation and overall well-being, which are crucial for muscle growth and fitness progress.
Regarding muscle growth specifically, while there is no direct evidence suggesting that finasteride inhibits muscle growth, the hormonal balance and side effects associated with its use could potentially hinder your fitness goals. If you experience any of the side effects mentioned, it may affect your energy levels, motivation, and overall performance in the gym, which could indirectly impact muscle growth.

In summary, while Proscar does not directly inhibit testosterone production, its effects on DHT and potential side effects related to sexual health could influence your fitness journey. If you are concerned about muscle growth while taking Proscar, it may be beneficial to consult with a healthcare professional or an endocrinologist. They can provide personalized advice based on your health status and fitness goals. Additionally, if you notice any adverse effects that impact your training, it may be worth discussing alternative treatments for your condition that do not carry the same risks.
Ultimately, maintaining a balanced diet, a proper workout regimen, and monitoring your body's responses to any medication will be key to achieving your muscle growth goals while managing any underlying health issues.
