Missed menstruation?
Hello, Doctor.
Since I started menstruating, my periods have always been quite regular, especially in the last two to three years, occurring approximately every twenty days.
However, my menstrual flow tends to be on the heavier side, bright red, and sometimes accompanied by large clots.
In the past, I experienced headaches before and during my periods, but this has not been an issue in the last two to three years; instead, I have started to experience mild dysmenorrhea.
I generally prefer to eat flavorful foods and drink beverages.
At the beginning of September, I moved to a new city for a new job, and due to work, I often have to change my living situation approximately every ten days.
My diet has also become somewhat irregular (the workplace provides breakfast, lunch, and dinner, but the chef tends to prepare spicy and fried dishes, making every meal spicy.
However, I have not been drinking iced beverages as I used to).
I have also been under some stress.
In mid-September, my period came a few days earlier than usual, but everything else seemed normal.
However, my period for October has not arrived yet.
Just before it was due, I experienced breast tenderness and developed several pimples on my forehead, but those symptoms subsided.
I have tried heat therapy, consuming motherwort, and ginger with hawthorn and brown sugar, but my period still has not come.
Currently, I feel persistent lower abdominal bloating and discomfort when pressed, along with lower back pain.
I have also started experiencing constipation, which is unusual for me, and I feel very dry in my mouth.
The possibility of pregnancy is virtually nonexistent: my last sexual encounter was at the end of August, and I had my period in September.
I also used over-the-counter pregnancy tests on October 27 and 30 (using different brands), both of which were negative.
I have considered that stress might be a factor, but even during the most severe episodes of depression and stress in the past, my periods remained regular, which makes me a bit worried.
Could you please advise on the following:
1.
Can pregnancy be ruled out?
2.
What can I do or take to induce my period or alleviate my discomfort?

Dr. Yan Sumei reply Traditional Chinese Medicine
Dear Anonymous Miss,
As you may have noticed, irregular menstruation could be caused by work-related stress and frequent changes in your environment over the past few weeks.
To rule out pregnancy, a home pregnancy test would be advisable.
However, I cannot prescribe medication to induce menstruation or alleviate discomfort without a proper pulse diagnosis and consultation to assess your condition of excess or deficiency, as well as cold or heat.
I recommend seeking assistance from a qualified Traditional Chinese Medicine practitioner for professional help.
Wishing you all the best and good health!
Sincerely,
Yen Su-Mei,

More Info
Understanding irregular menstruation from the perspective of Traditional Chinese Medicine (TCM) involves a holistic approach that considers various factors, including lifestyle, diet, emotional well-being, and physical health. In your case, several elements may be contributing to your irregular menstrual cycle, particularly the recent changes in your environment and lifestyle.
1. Possible Causes of Irregular Menstruation
From a TCM perspective, irregular menstruation can be attributed to several factors:
- Dietary Habits: Your preference for spicy and fried foods can lead to an imbalance in your body’s internal environment. In TCM, excessive heat from spicy foods can disrupt the flow of Qi (energy) and blood, potentially leading to symptoms like heavy menstrual flow and blood clots. Additionally, a diet lacking in cooling and nourishing foods may contribute to heat accumulation, which can affect menstrual regularity.
- Stress and Emotional Factors: Stress is known to impact hormonal balance and can lead to irregularities in the menstrual cycle. While you mentioned that you have experienced stress in the past without affecting your cycle, the cumulative effect of ongoing stress, especially with frequent changes in your living situation and job, may be influencing your current state.
- Qi and Blood Deficiency: Symptoms such as abdominal bloating, discomfort, and fatigue may indicate a deficiency in Qi and blood. In TCM, the spleen is responsible for the production of Qi and blood, and any disruption in its function can lead to irregular menstruation.
- Environmental Changes: Moving to a new city and adapting to a new job can introduce stressors that affect your body’s balance. The change in routine and diet can also lead to disruptions in your menstrual cycle.
2. Can Pregnancy Be Ruled Out?
Based on your description, it seems unlikely that pregnancy is the cause of your missed period, especially since you have had negative pregnancy tests and your last sexual encounter was in August. However, if you continue to experience symptoms or have concerns, it may be beneficial to consult a healthcare provider for further evaluation.
3. Remedies and Recommendations
To help regulate your menstrual cycle and alleviate discomfort, consider the following TCM approaches:
- Dietary Adjustments: Incorporate more cooling and nourishing foods into your diet, such as leafy greens, fruits, and whole grains. Avoid excessive spicy and fried foods. Herbal teas like chamomile or peppermint can also help soothe digestive discomfort.
- Herbal Remedies: While you have tried some herbal remedies like Yimucao (Motherwort) and ginger, it may be beneficial to consult a qualified TCM practitioner who can prescribe a personalized herbal formula tailored to your specific symptoms. Formulas like "Xuefu Zhuyu Tang" (Blood Stasis Removal Decoction) may help improve blood circulation and alleviate menstrual discomfort.
- Stress Management: Engage in stress-reducing activities such as yoga, meditation, or tai chi. These practices can help balance your emotions and improve overall well-being.
- Acupuncture: Consider acupuncture as a complementary treatment. It can help regulate menstrual cycles, alleviate pain, and promote relaxation.
- Lifestyle Modifications: Maintain a regular sleep schedule and engage in moderate exercise to promote overall health and well-being.
Conclusion
In summary, irregular menstruation can stem from various factors, including dietary habits, stress, and lifestyle changes. By making adjustments to your diet, managing stress, and potentially seeking TCM treatments, you may find relief and restore balance to your menstrual cycle. If symptoms persist or worsen, it is advisable to consult a healthcare professional for further evaluation and guidance.
