Nasal irrigation with symptoms of neck stiffness and pain + headache?

I apologize, but I would like to ask about a situation.
In October, I underwent surgery for turbinate hypertrophy and performed nasal irrigation for about three weeks.
Recently, I have experienced headaches in the right occipital region, and during nasal irrigation, I feel soreness and numbness in the right side of my head and face.
Currently, there is no blood in my nasal discharge, but there is a small amount of yellow secretion, and my nose does not hurt.
I have previously consulted a neurologist for my headaches and took medication, but there was no improvement, only a reduction in the intensity of the pain without any relief.
Could this be due to inflammation at the surgical site?

Xiao Hua, 30~39 year old female. Ask Date: 2017/12/10

Dr. Zheng Jueyi reply Otolaryngology

Hello, due to the surgery being several months ago, the yellow nasal discharge is often associated with infection.
Currently, the symptoms do not appear to be indicative of an infection.
It is recommended to return to the otolaryngology department to confirm whether there is sinusitis or another infection.
If not, it may be advisable to continue follow-up with the neurology outpatient clinic.
Wishing you peace and health.
Sincerely, Dr.

Post-surgery headaches and facial pain can be concerning, especially after undergoing a procedure like nasal surgery. In your case, it seems you had surgery for nasal turbinate hypertrophy, and now you are experiencing headaches and facial discomfort, particularly on the right side, along with some yellow nasal discharge.
First, it’s important to understand that after nasal surgery, it is not uncommon to experience some degree of discomfort, including headaches. This can be due to several factors:
1. Healing Process: After surgery, the nasal passages and surrounding tissues are healing. This healing process can lead to inflammation and swelling, which may contribute to headaches and facial pain.

2. Nasal Irrigation: While nasal irrigation is generally beneficial for clearing mucus and promoting healing, it can sometimes cause discomfort or exacerbate existing pain, especially if there is any residual swelling or irritation from the surgery.

3. Sinus Issues: The presence of yellow nasal discharge could indicate some level of sinus inflammation or infection, even if you do not feel significant pain in the nasal area. Sinusitis can lead to referred pain in the head and face, which might explain the headaches and facial discomfort you are experiencing.

4. Nerve Sensitivity: Surgery can sometimes lead to increased sensitivity in the nerves around the surgical site. This can manifest as pain or discomfort in the head and face, particularly if there is any irritation or inflammation affecting the trigeminal nerve, which supplies sensation to the face.

Given that you have already consulted a neurologist and have not found relief with medication, it may be beneficial to return to your ENT specialist for a thorough evaluation. They can assess whether there is any ongoing inflammation, infection, or other complications related to your surgery.
In terms of treatment, if there is evidence of sinusitis or other complications, your doctor may recommend:
- Nasal Corticosteroids: These can help reduce inflammation in the nasal passages and sinuses, potentially alleviating headaches and facial pain.

- Antibiotics: If there is a bacterial infection present, antibiotics may be necessary.

- Continued Nasal Irrigation: This can help keep the nasal passages clear, but it should be done gently to avoid further irritation.

- Pain Management: If over-the-counter medications have not been effective, your doctor may consider prescribing stronger pain relief options.

Additionally, if your symptoms persist despite treatment, further imaging studies such as a CT scan or MRI may be warranted to rule out any structural issues or complications that could be contributing to your symptoms.

In summary, while some degree of discomfort after nasal surgery is normal, persistent headaches and facial pain warrant further investigation. It’s crucial to work closely with your healthcare providers to identify the underlying cause and develop an appropriate treatment plan.
