Post-operative headache after septoplasty?
Hello doctor, my boyfriend recently underwent surgery for a deviated septum, and it has been 2 weeks since the procedure.
He has had his stitches removed and has been returning for weekly nasal cleaning.
Currently, his nose is not very painful, but he is experiencing significant tightness around the bridge of his nose and the area around his eyes, which has led to persistent headaches.
Initially, he took acetaminophen for pain relief, but there was no improvement.
After switching to cataflam, he experienced slight relief for 1-2 days, but today he has started experiencing severe headaches again.
I would like to ask the doctor if it is normal to have headaches after septoplasty? Why has the headache persisted for 2 weeks without improvement? Would more frequent nasal cleaning at the clinic help alleviate this issue? Thank you, doctor.

Hello: Generally, postoperative headaches are related to scabs, blood clots, or infections in the nasal cavity.
However, by two weeks post-surgery, most wounds should have healed.
If headaches persist, it is advisable to return to the original surgeon for an evaluation to determine if there are other underlying issues.
Wishing you safety and health.
Sincerely, Dr.

Post-surgery headaches, particularly after a septoplasty, can be a common concern for many patients. Your boyfriend's experience of persistent headaches two weeks after the surgery, despite the initial pain subsiding, raises several important considerations.
Firstly, it is essential to understand that headaches following nasal surgery can arise from various factors. One of the primary reasons for headaches post-septoplasty is the presence of swelling and inflammation in the nasal passages and surrounding tissues. The surgical procedure involves manipulating the nasal structures, which can lead to irritation of the surrounding nerves and tissues, resulting in discomfort and headaches. This inflammation can take time to resolve, and it is not uncommon for patients to experience headaches for several weeks after surgery.
Another potential cause of headaches after septoplasty is the formation of crusts or scabs inside the nasal passages. These can occur as the surgical site heals, and they may lead to increased pressure and discomfort, contributing to headache symptoms. If your boyfriend is experiencing tightness around the forehead and eyes, it could be related to sinus pressure or tension headaches, which are often exacerbated by nasal congestion or irritation.
Additionally, the use of pain medications, such as acetaminophen and cataflam, may provide temporary relief but might not address the underlying cause of the headache. If the headaches are severe and persistent, it is crucial to consult with the surgeon or an ENT specialist to rule out any complications, such as infection or improper healing. They may recommend further evaluation, including imaging studies, to ensure that there are no underlying issues contributing to the headaches.
Regarding the frequency of nasal cleaning, while it is essential to maintain nasal hygiene post-surgery, excessive cleaning can sometimes irritate the nasal passages further and may not necessarily lead to improvement in headache symptoms. It is best to follow the surgeon's recommendations regarding nasal care and cleaning frequency.
In summary, while it is not unusual to experience headaches after septoplasty, especially around the forehead and eyes, it is essential to monitor the symptoms closely. If the headaches persist or worsen, or if there are additional symptoms such as fever, increased nasal discharge, or changes in vision, it is crucial to seek medical attention promptly. The healthcare provider may consider adjusting pain management strategies, recommending nasal sprays to reduce inflammation, or exploring other therapeutic options to alleviate the headaches effectively.
Encourage your boyfriend to communicate openly with his healthcare provider about his symptoms and any concerns he may have. Proper follow-up care is vital to ensure a smooth recovery and to address any complications that may arise after surgery.
