Chest pain?
Doctor, I would like to ask, last night while sleeping on my side, I felt discomfort in my left chest when breathing, and it felt like my breathing was not very smooth.
I have not felt well all day today, often needing to take deep breaths, and I experience sharp pain in my left chest when I laugh or cough.
What could be the cause? Do I need to undergo any tests? I have never experienced this before.
Thank you for your help, I appreciate it.

Dear A-Jing,
According to the letter, you should promptly visit the hospital for an examination.
The physician will determine whether a chest X-ray or an electrocardiogram (ECG) is necessary for further treatment.

Chest pain can be a concerning symptom, and it is essential to understand its potential causes and when to seek medical attention. Based on your description, you experienced discomfort in your left chest while sleeping on your side, which has persisted throughout the day. You also mentioned that laughing and coughing exacerbate the pain.
There are several potential causes for chest pain, and they can range from benign to serious conditions. Here are some common causes:
1. Musculoskeletal Issues: Given that you mentioned the pain worsens with movement (like laughing and coughing), it could be related to musculoskeletal problems, such as muscle strain or costochondritis (inflammation of the cartilage connecting the ribs to the sternum). This type of pain is often sharp and can be reproduced by pressing on the affected area.
2. Respiratory Conditions: Conditions such as pleuritis (inflammation of the lining of the lungs) or pneumonia can cause chest pain, especially if it is associated with breathing difficulties. If you have a cough or any signs of respiratory infection (fever, chills, or difficulty breathing), this could be a possibility.
3. Cardiac Issues: While less likely given your age and the nature of the pain, it is crucial to rule out cardiac causes, especially if you have risk factors such as a family history of heart disease, smoking, or high blood pressure. Cardiac pain is often described as a pressure or squeezing sensation and may radiate to the arm, neck, or jaw.
4. Gastrointestinal Causes: Conditions such as gastroesophageal reflux disease (GERD) or esophageal spasms can cause chest pain that may mimic cardiac pain. This type of pain is often associated with eating or lying down.
5. Anxiety or Panic Attacks: Anxiety can manifest physically, leading to chest discomfort, shortness of breath, and other symptoms. If you have been under stress or experiencing anxiety, this could be a contributing factor.
Given your symptoms, it is advisable to seek medical attention, especially since the pain is persistent and worsens with certain movements. A healthcare provider may recommend several diagnostic tests, including:
- Physical Examination: A thorough examination to assess the nature of the pain and any associated symptoms.
- Electrocardiogram (ECG): To evaluate your heart's electrical activity and rule out any cardiac issues.
- Chest X-ray: To check for any lung-related issues or structural abnormalities.
- Blood Tests: To assess for markers of inflammation or cardiac enzymes if a heart issue is suspected.
In summary, while there are several potential causes for your chest pain, it is essential to consult a healthcare professional for a proper evaluation. They can help determine the underlying cause and recommend appropriate treatment. If you experience severe pain, difficulty breathing, or any other alarming symptoms, do not hesitate to seek emergency medical care. Your health and safety are paramount, and timely intervention can make a significant difference.
