Urinary symptoms
Hello Dr.
Chen: I would like to ask a question.
I have never had sexual intercourse and rarely masturbate; it has been a year since I last masturbated.
Previously, I saw a doctor who diagnosed me with varicocele, and the doctor said it was not a concern and that I could consider surgery if needed.
After about a month, I noticed a few symptoms:
1.
Sometimes when I strain during bowel movements, I expel a white fluid, and at times, I experience a burning sensation.
(What could this symptom indicate?)
2.
I wake up during sleep due to nocturnal emissions, and during ejaculation, I feel a slight burning sensation and pain (it feels like it's in the area between the anus and the genitals).
(What could this symptom indicate?)
These are the main symptoms I am experiencing.
I am a bit anxious and unsure about what illness I might have.
Could you please advise me? Thank you.

Hello! Thank you for reaching out with your concerns regarding your urological symptoms. Based on the symptoms you've described, it seems that you are experiencing a few different issues that could be related to your previous diagnosis of varicocele (enlarged veins within the scrotum) and possibly other underlying conditions. Let’s break down your symptoms and discuss what they might indicate.
1. Urination of White Fluid During Bowel Movements: The occurrence of a white fluid during bowel movements, especially when straining, could be related to several factors. One possibility is that this fluid is seminal fluid, which can sometimes be expelled due to increased abdominal pressure during straining. This can happen in individuals with varicocele, as the condition can affect the normal function of the reproductive system. The burning sensation you mentioned could be due to irritation or inflammation in the urethra or prostate, possibly indicating prostatitis (inflammation of the prostate gland) or another type of infection.
2. Ejaculation with Burning Sensation and Pain: The burning sensation and pain you experience during ejaculation, particularly if it radiates from the genital area to the anal region, could suggest several conditions. One common issue is prostatitis, which can cause discomfort during ejaculation and may also lead to a feeling of heat or burning. Another possibility is that you may have a condition known as pelvic floor dysfunction, where the muscles in the pelvic area are tense or not functioning properly, leading to pain during sexual activity or ejaculation.
Given these symptoms, it is crucial to consult a urologist or a healthcare provider who specializes in male reproductive health. They can conduct a thorough examination, which may include a physical exam, urine tests, and possibly imaging studies to assess the condition of your prostate and any other relevant structures.
In addition to the physical examination, it would be beneficial to discuss your medical history, including your varicocele diagnosis and any other health issues you may have experienced. It’s also important to mention any lifestyle factors, such as stress levels, diet, and exercise, as these can all impact urological health.
While it’s understandable to feel anxious about these symptoms, many urological issues are treatable, and early intervention can lead to better outcomes. Your healthcare provider will be able to provide you with a clear diagnosis and discuss potential treatment options, which may range from lifestyle modifications to medications or surgical interventions if necessary.
In the meantime, try to keep track of your symptoms, including when they occur and any activities that seem to exacerbate them. This information can be very helpful for your healthcare provider in determining the best course of action.
Remember, you are not alone in this, and seeking help is a positive step towards understanding and managing your health. Don't hesitate to reach out for professional medical advice, as they will be able to provide you with the most accurate information tailored to your specific situation. Thank you for your question, and I wish you the best in your journey towards better health!
