Glans issues
Hello doctor, I would like to ask about my issue with phimosis.
Recently, I started to gradually retract the foreskin during my showers.
Today, for the first time, I tried not to pull the foreskin back over the glans to let it adapt to the friction from my clothing.
However, after about an hour, I noticed some peeling on the glans.
Is this normal? Thank you.

Hello: You might want to take it slow.
If you previously had a full wrap and rarely removed it, try gently cleaning the area and drying it off during your daily showers.
After a few weeks, you can attempt to remove it again, as the skin may currently be too sensitive.
This could lead to redness and peeling.
Wishing you good health.

When it comes to the sensitivity of the glans and the experience of peeling after retracting the foreskin, it is important to understand the anatomy and physiology of the penis, particularly in the context of individuals who have not previously exposed the glans to external stimuli.
The glans penis, or the head of the penis, is typically covered by the foreskin in uncircumcised males. This foreskin serves several functions, including protecting the sensitive glans from friction and irritation. When the foreskin is retracted for the first time, or when it is kept retracted for extended periods, the glans can become exposed to new sensations, including friction from clothing, which it may not be accustomed to.
Peeling or flaking of the skin on the glans can occur for several reasons. One common cause is irritation due to friction, especially if the glans has been covered for a long time and is suddenly exposed to clothing or other surfaces. This irritation can lead to a mild inflammatory response, resulting in peeling or flaking of the skin. Additionally, if there is any moisture trapped under the foreskin prior to retraction, it can lead to maceration of the skin, contributing to peeling.
Another possibility is that the skin on the glans is adjusting to the new exposure. The skin may be sensitive and react to the friction and changes in moisture levels, leading to peeling as the outer layer of skin sheds. This is a normal process as the skin adapts to its new environment.
However, it is essential to differentiate between normal peeling and signs of a potential problem. If the peeling is accompanied by redness, swelling, pain, or any discharge, it could indicate an infection or another dermatological condition, such as balanitis (inflammation of the glans) or a fungal infection. In such cases, it is advisable to consult a healthcare professional for an accurate diagnosis and appropriate treatment.
To manage sensitivity and peeling, consider the following recommendations:
1. Gradual Exposure: If you are trying to acclimate your glans to friction, do so gradually. Start with short periods of exposure and gradually increase the time as your sensitivity decreases.
2. Moisturization: Using a gentle, unscented moisturizer can help keep the skin hydrated and reduce irritation. Avoid products with harsh chemicals or fragrances that could exacerbate sensitivity.
3. Hygiene: Maintain good hygiene by cleaning the area gently with mild soap and water. Avoid excessive scrubbing, which can irritate the skin further.
4. Clothing Choices: Opt for loose-fitting underwear and pants to minimize friction against the glans. This can help reduce irritation during the adjustment period.
5. Monitor Symptoms: Keep an eye on the peeling. If it persists or worsens, or if you experience any other concerning symptoms, seek medical advice.
In summary, while some peeling after retracting the foreskin can be normal, it is essential to monitor the situation closely. If there are any signs of infection or if the peeling does not resolve, consulting with a healthcare provider is the best course of action. They can provide personalized advice and treatment options based on your specific situation.
