Coughing issues

Hello Doctor, I have been coughing for over a month.
I have seen many doctors, and there has only been slight improvement.
However, it is strange that whenever I drink cold beverages during the day, I end up coughing non-stop in the evening.
The phlegm I cough up is yellow, but if I don’t drink cold beverages, I don’t cough persistently at night.
I would like to ask if this could be due to nasal congestion causing post-nasal drip, which irritates the throat nerves and leads to my persistent cough.
I understand that coughing is a protective mechanism of the body.
I have been experiencing headaches due to this symptom for a long time, and the constant coughing is quite uncomfortable.
As a vocational high school student, I am exposed to automotive spray paint, and the solvent we use, banana oil, is an organic solvent that can be harmful to the trachea and lungs.
I always wear a specialized respirator while spraying paint, but I am concerned that my exposure, combined with my pre-existing respiratory issues, might be causing these symptoms.
Thank you for taking the time to respond.
(Note: I do not smoke.)

Dr. Zheng Jueyi reply Otolaryngology

Hello, if drinking cold beverages easily triggers coughing, it is often related to bronchial sensitivity, such as asthma.
Other common causes of chronic cough may be associated with conditions like sinusitis leading to post-nasal drip or gastroesophageal reflux disease (GERD).
If the symptoms persist without improvement, it is advisable to consult an otolaryngologist or a pulmonologist for further evaluation.
Wishing you safety and health.
Sincerely, Dr.

More Info

Persistent cough can be a frustrating and debilitating condition, especially when it interferes with daily activities and sleep. Based on your description, it seems that your cough has been ongoing for over a month, with specific triggers such as consuming cold beverages. This could suggest a few underlying issues that may be contributing to your symptoms.

Possible Causes of Your Persistent Cough
1. Postnasal Drip: Given your history of nasal congestion, it is quite possible that you are experiencing postnasal drip. This occurs when mucus from the nasal passages drips down the back of the throat, irritating it and triggering a cough reflex. Cold beverages can sometimes exacerbate this condition by causing the mucus to thicken, leading to increased irritation.

2. Allergic Rhinitis: If you have a history of allergies, this could also be a contributing factor. Allergens can cause inflammation in the nasal passages, leading to increased mucus production and postnasal drip. This is particularly relevant if you notice that your symptoms worsen in certain environments or seasons.

3. Irritants: Your exposure to organic solvents, such as those found in paint, could also be a significant factor. Even with protective gear, prolonged exposure can lead to respiratory irritation, which may manifest as a chronic cough. The chemicals in these products can irritate the airways, leading to inflammation and increased mucus production.

4. Asthma: Given your symptoms, it may also be worth considering asthma as a potential cause. Asthma can present with a chronic cough, especially at night or after exposure to certain triggers, such as cold air or irritants. If you have a family history of asthma or allergies, this could further support this possibility.

5. Infections: While you mentioned that you have seen multiple doctors, it’s important to rule out any lingering infections, such as bronchitis or sinusitis, which can cause prolonged coughing. Yellow phlegm can indicate a bacterial infection, although it can also occur with viral infections.

Remedies and Management Strategies
1. Hydration: Staying well-hydrated can help thin mucus, making it easier to expel. Warm fluids, such as herbal teas or broths, can be particularly soothing.

2. Humidification: Using a humidifier in your room can help keep the air moist, which may alleviate coughing, especially at night. Dry air can exacerbate coughs and irritate the throat.

3. Avoiding Triggers: Since you’ve identified that cold drinks trigger your cough, it may be beneficial to avoid them. Additionally, minimizing exposure to irritants, such as paint fumes, is crucial. Ensure that you are working in a well-ventilated area and consider using additional protective measures if necessary.

4. Nasal Irrigation: Saline nasal sprays or rinses can help clear mucus from your nasal passages, reducing postnasal drip and associated coughing.

5. Over-the-Counter Medications: Depending on your symptoms, decongestants or antihistamines may provide relief. However, it’s essential to consult with a healthcare professional before starting any new medication.

6. Consulting a Specialist: If your symptoms persist despite these measures, it may be beneficial to see an ear, nose, and throat (ENT) specialist or a pulmonologist. They can perform more detailed evaluations, such as allergy testing or pulmonary function tests, to determine the underlying cause of your cough.

Conclusion
Persistent cough can stem from various causes, including postnasal drip, allergies, irritants, or asthma. Identifying and avoiding triggers, maintaining hydration, and using humidification can help manage symptoms. However, if your cough continues or worsens, seeking further evaluation from a specialist is advisable to rule out any serious underlying conditions. Remember, your health is paramount, and addressing these symptoms early can lead to better outcomes.
