Is the bleeding stopped? Can I continue to take it?

Hello, Dr.
Yan.
I am 48 years old.
Recently, my menstrual period lasted for over ten days with continuous spotting.
I consulted a gynecologist who administered progesterone, indicating it would help regulate my cycle, and indeed, it cleared up.
However, the next day, I felt like my period was about to start again, and it did, with more spotting.
I then switched to a traditional Chinese medicine practitioner who prescribed a modified Xiao Yao San, which includes Rehmannia, Scrophularia, White Peony, Ophiopogon, and Lycium bark.
Now my menstrual flow has cleared up, and I continue to see the TCM doctor, who has prescribed the same herbs along with Sheng Yu Tang and an out-of-pocket medication, Ejiao.

I would like to ask the following questions:
1.
Why did I experience another menstrual cycle just one day after the progesterone injection?
2.
Since my menstrual flow has cleared, will continuing to take these herbs affect my body? Could it potentially cause my period to stop (the last TCM doctor clearly stated that these herbs are for stopping bleeding, yet now I am advised to continue taking them)?
3.
I have several uterine fibroids that are less than 3 cm in size.
Will taking these prescribed herbs cause the fibroids to grow?
4.
I often feel a sense of fullness in my chest.
Is this due to Qi stagnation?

Dr. Yan Sumei reply Traditional Chinese Medicine

Dear online friend,
Thank you for your letter.
Regarding the issue of menstruation returning a day after receiving a progesterone injection, this should be discussed with your original obstetrician-gynecologist.
Since your menstrual flow has cleared, you can continue taking the modified Xiao Yao San, which includes Rehmannia, Scrophularia, White Peony, Ophiopogon, and Lycium bark, along with Sheng Yu Decoction and Ejiao.
Will this cause your menstruation to stop? The original prescription has been supplemented with some Qi and blood tonifying herbs, which aim to prevent excessive bleeding in the future and replenish the lost Qi and blood; this medication will not cause your menstruation to cease.
As for the presence of several uterine fibroids measuring less than 3 cm, will taking the aforementioned herbs cause the fibroids to grow? The likelihood is low, as these herbal medicines are not known to have such effects.
Is the feeling of breast fullness due to Qi stagnation? It is possible; sometimes this sensation occurs before menstruation or during hormonal imbalances associated with menopause.
Wishing you health and peace!
Yours sincerely,
Yen Su-Mei

More Info

When dealing with menstrual irregularities and the use of herbal medicine, it is essential to approach the situation with caution and a clear understanding of the body's responses. Based on your description, you have experienced prolonged bleeding, which was addressed with hormonal treatment (progesterone) and subsequently with traditional Chinese medicine (TCM). Here are some insights regarding your questions:
1. Why did the injection of progesterone not stop the bleeding?
Progesterone is often used to help regulate the menstrual cycle and can assist in stopping abnormal bleeding by stabilizing the endometrial lining. However, individual responses to hormonal treatments can vary significantly. Factors such as the underlying cause of the bleeding (e.g., hormonal imbalances, fibroids, or other conditions) can influence how effective the treatment is. If the bleeding resumed shortly after the injection, it might indicate that the underlying issue was not fully addressed or that your body requires additional time or different treatment modalities to stabilize.

2. Should you continue taking the herbal medicine after the bleeding has stopped?
Since your bleeding has ceased, continuing the herbal medicine prescribed by your TCM practitioner may be beneficial, especially if it is intended to support overall health and balance. However, it is crucial to communicate with your healthcare provider about the ongoing use of these herbs. While some herbal remedies can help with menstrual regulation and support uterine health, others may have effects that could lead to further complications if not monitored properly. It is essential to ensure that the herbs you are taking do not conflict with any other medications or treatments you are undergoing.

3. Will the herbal medicine affect the size of your uterine fibroids?
The relationship between herbal medicine and uterine fibroids is complex. Some herbs may have properties that can help manage symptoms associated with fibroids, but there is limited scientific evidence regarding their direct impact on fibroid size. It is advisable to discuss this with your TCM practitioner, as they can provide insights based on your specific condition and the herbs being used. Regular monitoring through ultrasound or other imaging techniques may be necessary to assess any changes in the fibroids' size.

4. Is the breast tenderness you are experiencing related to Qi stagnation?
In TCM, breast tenderness can be associated with Qi stagnation, particularly if there are accompanying symptoms such as mood swings or digestive issues. However, it can also be related to hormonal fluctuations, especially during or after menstruation. If the tenderness persists or worsens, it would be prudent to consult with your healthcare provider to rule out any other potential causes, including hormonal imbalances or other medical conditions.

In summary, while it is generally acceptable to continue herbal medicine after bleeding has stopped, it is crucial to do so under the guidance of qualified healthcare professionals. Regular follow-ups with both your gynecologist and TCM practitioner can help ensure that your treatment plan is safe and effective. Additionally, monitoring your symptoms and any changes in your condition will provide valuable information for your healthcare providers to make informed decisions about your ongoing treatment. Always prioritize open communication with your healthcare team to achieve the best outcomes for your health.
