Pain in the lateral ribs?
Hello doctor, I have ankylosing spondylitis and have been taking the anti-inflammatory medication Mobic, one tablet daily, for a long time.
I have noticed that the left side of my rib cage, just below the armpit, hurts when pressed.
It doesn't hurt when not touched, but even slight pressure causes pain.
I tried pressing a similar spot on the right side, and it does not hurt.
I previously asked a rheumatologist, and he mentioned that ankylosing spondylitis rarely affects that area.
What could be the cause of the pain when pressing on that spot?

It may be that the pleura is slightly inflamed; you can get an X-ray to check for any bone issues.

Left side rib pain can be a concerning symptom, especially for someone with a history of ankylosing spondylitis (AS) like yourself. Ankylosing spondylitis is a type of inflammatory arthritis that primarily affects the spine and the sacroiliac joints in the pelvis. While it typically causes pain and stiffness in the back, it can also lead to discomfort in other areas, including the ribs.
Given your description of the pain being localized to the left side near the ribcage, it is essential to consider several potential causes:
1. Musculoskeletal Issues: The pain you experience when pressing on the left side of your ribcage could be related to muscle strain or inflammation of the costal cartilage, which connects the ribs to the sternum. This condition, known as costochondritis, can cause localized pain that worsens with pressure or movement. It is often benign and can be treated with rest, ice, and over-the-counter pain relief.
2. Referred Pain: Sometimes, pain in the rib area can be referred from other structures. For instance, issues with the spine, such as herniated discs or facet joint dysfunction, can lead to pain that radiates to the ribcage. Given your history of ankylosing spondylitis, it is possible that inflammation in the spine could be contributing to your rib pain.
3. Inflammation: Since you are on Mobic (meloxicam), a non-steroidal anti-inflammatory drug (NSAID), it is crucial to monitor how well it manages your overall inflammation. If your AS is not well-controlled, it could lead to increased pain in various areas, including the ribs.
4. Internal Organ Issues: Although less likely, it is essential to rule out any issues related to internal organs. Conditions affecting the lungs, heart, or gastrointestinal tract can sometimes present as rib pain. For example, pleuritis (inflammation of the lining of the lungs) can cause sharp pain that worsens with deep breaths or coughing.
5. Nerve Compression: Conditions such as thoracic outlet syndrome or intercostal neuralgia can lead to pain in the rib area due to nerve compression or irritation. This type of pain may be sharp and can be exacerbated by certain movements or positions.
Given that your rheumatologist has indicated that ankylosing spondylitis rarely affects the area you are describing, it would be prudent to seek further evaluation. A thorough physical examination, possibly including imaging studies such as X-rays or MRI, may be necessary to determine the exact cause of your pain.
In the meantime, consider the following recommendations:
- Rest and Avoid Aggravating Activities: If certain movements or activities exacerbate your pain, try to avoid them until you have a clearer understanding of the cause.
- Pain Management: Continue using Mobic as prescribed, but also consider discussing with your doctor the possibility of adjunct therapies, such as physical therapy, which can help strengthen the muscles around the ribcage and improve flexibility.
- Monitor Symptoms: Keep track of any changes in your symptoms, including the intensity of the pain, any new symptoms, or changes in your overall health. This information will be valuable for your healthcare provider.
In conclusion, while rib pain can be attributed to various causes, it is essential to approach it with a comprehensive evaluation, especially considering your existing condition of ankylosing spondylitis. Consulting with your healthcare provider for a detailed assessment will help ensure appropriate management and treatment.
