Premature ejaculation / Pearly penile papules
Hello doctor, I previously had normal sexual function, but ever since I developed pearly penile papules in the sulcus of the glans (which I have confirmed with a doctor), that area has become particularly sensitive.
Now, whenever there is contact during intercourse or oral sex, it becomes extremely sensitive (other areas are not affected), leading to a bit of premature ejaculation.
I exercise regularly and do not smoke or drink.
What methods can I use to improve this situation? Thank you.

Pearl lesions are actually present in more than half of adult males (according to my clinic statistics), so there is no need for special concern.
The area you are particularly sensitive about is the line beneath the pearl lesions (the coronal sulcus), which is naturally a sensitive zone for men and can easily lead to ejaculation upon stimulation.
Gentle handling can help avoid excessive stimulation in that area; otherwise, medication may be necessary.

More Info
Managing sensitivity and premature ejaculation, especially after the appearance of pearly penile papules (PPP), can be a concern for many individuals. Pearly penile papules are benign, small, dome-shaped bumps that typically appear around the corona of the glans penis. They are a normal anatomical variation and are not associated with any disease or condition. However, their presence can sometimes lead to heightened sensitivity in the area, which may contribute to premature ejaculation.
Understanding Sensitivity and Premature Ejaculation
1. Heightened Sensitivity: The sensitivity you are experiencing in the area where the pearly penile papules are located may be due to the increased nerve endings in that region. This heightened sensitivity can lead to discomfort during sexual activities, resulting in difficulty controlling ejaculation.
2. Premature Ejaculation: This condition is defined as ejaculation that occurs with minimal sexual stimulation, often before the individual wishes it to happen. It can be caused by a variety of factors, including psychological issues (such as anxiety), physical sensitivity, or even hormonal imbalances.
Management Strategies
Here are several strategies that may help you manage sensitivity and premature ejaculation:
1. Desensitizing Products: Over-the-counter desensitizing creams or sprays can be applied to the penis before sexual activity. These products typically contain lidocaine or benzocaine, which can help reduce sensitivity and delay ejaculation. However, it's essential to use these products as directed and consult with a healthcare provider if you have concerns.
2. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can be beneficial. The stop-start method involves stimulating the penis until you feel close to ejaculation, then stopping until the urge decreases. The squeeze technique involves squeezing the base of the penis to reduce the urge to ejaculate. Practicing these techniques can help you gain better control over your ejaculation.
3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. To perform Kegel exercises, identify the muscles you use to stop urination. Contract these muscles for a few seconds, then relax. Repeat this several times a day.
4. Mindfulness and Relaxation Techniques: Anxiety and stress can exacerbate premature ejaculation. Mindfulness practices, such as meditation or deep-breathing exercises, can help you relax and reduce anxiety during sexual activity.
5. Consultation with a Specialist: If the problem persists, consider consulting a urologist or a sexual health specialist. They can provide tailored advice and may suggest other treatment options, such as therapy or medications specifically designed to address premature ejaculation.
6. Communication with Partner: Open communication with your partner about your concerns and experiences can help alleviate anxiety and create a supportive environment. This can also lead to exploring different sexual techniques that may be more comfortable for you.
7. Avoiding Irritants: Ensure that you are not using any products that may irritate the sensitive area, such as harsh soaps or lotions. Stick to gentle, hypoallergenic products to maintain skin health.
Conclusion
While the presence of pearly penile papules is not harmful, the sensitivity they may cause can lead to challenges in sexual performance. By employing a combination of desensitizing techniques, behavioral strategies, and possibly seeking professional help, you can effectively manage sensitivity and premature ejaculation. Remember, sexual health is an essential aspect of overall well-being, and addressing these concerns can lead to a more satisfying sexual experience. If symptoms persist or worsen, do not hesitate to seek medical advice for further evaluation and management.
