Dry eye symptoms following laser repair of retinal tears?
Dear Director Gao,
I have myopia of -600 diopters.
After a dilated fundus examination of my right eye, I was diagnosed with a very small tear located at the 6 o'clock position in the lower part of the retina.
On the same day of the outpatient visit, I underwent laser repair.
During the procedure, one of the laser shots caused me extreme pain, which was completely different from the sensation of minor cuts.
It has been 10 days since the laser treatment, and my right eye has been persistently dry and uncomfortable.
When I lie flat to sleep or wake up in the morning, my right eye feels particularly dry and painful.
While reading, the area where the laser was applied also experiences itching and stinging sensations.
These symptoms were never present before the laser treatment, and the stinging sensation in my right eye has not diminished over time.
Currently, my right eye's vision is normal, and there is no redness or swelling; however, the dryness and stinging sensation following the laser treatment have persisted for 10 days.
I would like to ask you, Director Gao, if there are clinical cases of dry eye syndrome occurring after laser repair of retinal tears.
If so, how long does it typically take for this symptom to fully improve? Thank you very much for taking the time to respond to my questions.
^_^

Hello, Ms.
Gao: The dry eye syndrome caused by retinal laser treatment should not be severe.
If you are experiencing dryness, it may be due to minor corneal abrasions.
It is advisable to consult an ophthalmologist for appropriate medication, which should help alleviate the symptoms.
The ophthalmology department at Puzi Hospital in Taiwan is open Monday to Friday from 8:00 AM to 12:00 PM and Wednesday afternoons from 1:00 PM to 4:30 PM.
Please note that online responses are for reference only; it is essential to seek medical attention at a healthcare facility if necessary.
I hope to have the opportunity to assist you.
Sincerely, Gao Pilin.

More Info
After undergoing laser repair for a retinal hole, it is not uncommon for patients to experience symptoms of dry eye, as you have described. The laser procedure itself primarily targets the retina, but the surrounding tissues and the eye's surface can also be affected. Here’s a detailed explanation of what you might expect and some potential solutions to alleviate your symptoms.
Understanding Dry Eye Symptoms Post-Laser Surgery
1. Causes of Dry Eye After Laser Surgery:
- Surgical Trauma: The laser procedure can cause temporary inflammation and irritation to the ocular surface, leading to a disruption in the tear film. This can result in symptoms of dryness, burning, and discomfort.
- Nerve Damage: Laser treatments can sometimes affect the corneal nerves, which play a crucial role in tear production and sensation. This can lead to a decrease in tear production and an increase in dry eye symptoms.
- Environmental Factors: Post-surgery, patients may be more sensitive to environmental factors such as air conditioning, wind, or prolonged screen time, which can exacerbate dry eye symptoms.
2. Symptoms You May Experience:
- Persistent dryness and discomfort in the affected eye.
- Sensations of burning, itching, or a gritty feeling.
- Increased tearing, which may seem counterintuitive but can occur as a reflex to dryness.
- Difficulty with visual tasks, such as reading or using a computer, due to discomfort.
Management and Treatment Options
1. Artificial Tears:
- Regular use of preservative-free artificial tears can help maintain moisture on the eye's surface. It’s advisable to use them frequently, especially during activities that require prolonged visual attention.
2. Warm Compresses:
- Applying warm compresses to the eyes can help stimulate the meibomian glands, which produce the oily layer of the tear film. This can improve overall tear quality and reduce dryness.
3. Avoiding Irritants:
- Try to minimize exposure to dry or windy environments. If you are in an air-conditioned space, consider using a humidifier to maintain moisture in the air.
4. Taking Breaks:
- Implement the 20-20-20 rule: every 20 minutes, take a 20-second break to look at something 20 feet away. This can help reduce eye strain and discomfort.
5. Consulting an Eye Care Specialist:
- If symptoms persist, it may be beneficial to consult with your ophthalmologist or an eye care specialist. They may recommend additional treatments, such as prescription eye drops (e.g., cyclosporine A or lifitegrast) or punctal plugs to help retain tears.
Prognosis
In clinical practice, it is recognized that dry eye symptoms can occur after retinal laser surgery, and while many patients experience improvement over time, the duration of symptoms can vary. Some patients may notice a gradual resolution of symptoms within a few weeks, while others may take longer. It is essential to maintain communication with your healthcare provider regarding your symptoms, as they can provide tailored advice and treatment options based on your specific situation.
Conclusion
While experiencing dry eye symptoms after laser repair of a retinal hole can be distressing, it is a recognized phenomenon. With appropriate management strategies, many patients find relief from their symptoms. If your symptoms do not improve or worsen, it is crucial to follow up with your eye care provider to explore further treatment options. Remember, your comfort and eye health are paramount, and there are solutions available to help you navigate this post-operative phase.
