Suspected urethral stricture?

Hello Doctor, I have been experiencing a sensation of incomplete bladder emptying every time I urinate, which has been bothering me for a long time (about 4 years).
Although it is not as severe as being unable to urinate, it is still uncomfortable.
I have visited the hospital twice for a simple check-up, but the doctors said it is likely just a sensation since my post-void residual volume is zero, so it shouldn't be a problem related to benign prostatic hyperplasia.
I suspect it could be urethral stricture, possibly due to mild inflammation of the urethra.
Could masturbation be a contributing factor? I would like to ask you, Doctor, is this urethral stricture? If it is mild, can it be completely cured?

Xiansheng, 20~29 year old female. Ask Date: 2018/01/21

Hello: If there is a urethral stricture, a cystoscopy is necessary to confirm the diagnosis; it cannot be diagnosed without examination.
What can be done is to pay attention to lifestyle, ensuring that exercise and diet are normal, and absolutely avoiding certain beverages.
If it becomes unavoidable, then a cystoscopy or urodynamic study may be performed, as these are invasive procedures that involve inserting a catheter into the urethra and into the bladder, which can be uncomfortable.
Wishing you good health.

More Info

Urethral stricture is a condition characterized by the narrowing of the urethra, which can lead to various urinary symptoms, including the sensation of incomplete bladder emptying, difficulty urinating, and urinary tract infections. Based on your description of experiencing a feeling of residual urine after urination for about four years, it is indeed possible that you may be dealing with a urethral stricture or another underlying issue affecting your urinary function.

Symptoms of Urethral Stricture
The symptoms you are experiencing, such as the sensation of incomplete emptying, can be indicative of urethral stricture. Other common symptoms include:
- Weak or interrupted urine stream
- Difficulty starting urination
- Frequent urination or urgency
- Pain during urination
- Recurrent urinary tract infections

Potential Causes
Urethral strictures can arise from various causes, including:
- Trauma: Injury to the urethra from accidents or surgical procedures.

- Inflammation: Conditions such as urethritis (inflammation of the urethra) can lead to scarring and narrowing.

- Infections: Sexually transmitted infections (STIs) or other infections can cause inflammation and scarring.

- Congenital conditions: Some individuals may be born with a narrowed urethra.

- Iatrogenic causes: Previous medical procedures, such as catheterization, can sometimes lead to strictures.

You mentioned that you have experienced mild urethral inflammation and have engaged in masturbation. While masturbation itself is generally not a direct cause of urethral stricture, any activity that causes trauma or irritation to the urethra could potentially contribute to inflammation or scarring over time.

Diagnosis
To confirm a diagnosis of urethral stricture, a urologist may perform several diagnostic tests, including:
- Uroflowmetry: Measures the flow rate of urine to assess for any obstructions.

- Cystoscopy: A procedure where a thin tube with a camera is inserted into the urethra to visualize any narrowing or abnormalities.

- Imaging studies: Such as ultrasound or MRI, may be used to assess the urinary tract.

Treatment Options
If urethral stricture is confirmed, treatment options may include:
1. Dilation: A procedure to widen the narrowed section of the urethra using a balloon or other instruments.

2. Urethrotomy: A surgical procedure to cut the stricture and allow for normal urine flow.

3. Urethroplasty: A more extensive surgical procedure that involves removing the narrowed segment of the urethra and reconstructing it.

4. Self-Catheterization: In some cases, patients may be taught to self-catheterize to help manage symptoms.

Conclusion
Given your symptoms and the duration of your discomfort, it is advisable to seek a consultation with a urologist who specializes in urinary tract disorders. They can provide a thorough evaluation and recommend appropriate diagnostic tests to determine if you have a urethral stricture or another underlying condition. Early diagnosis and treatment can help alleviate your symptoms and improve your quality of life.
In the meantime, it is essential to maintain good hydration and avoid any irritants that may exacerbate your symptoms. If you experience any worsening symptoms, such as pain, fever, or blood in your urine, seek medical attention promptly.
