Skin Tears on the Scrotum and Foreskin: Treatment Options - Urology

Tearing of the foreskin/scrotum?

A year ago, I had a skin abrasion at the base of my scrotum and on my foreskin due to tight clothing.
At that time, the doctor prescribed a topical ointment.
Two days ago, I noticed another abrasion at the base of my scrotum, so I applied the same ointment.
However, the next evening, I developed another abrasion under the frenulum of my foreskin.
I went to the hospital for a check-up the following morning, but the examination seemed inconclusive.
The doctor prescribed Meikang ointment, but I just read some articles online suggesting that it might not be appropriate to apply Meikang ointment on an abrasion.
Should I seek a second opinion from another doctor? Thank you for your help.

Hello: The skin on the scrotum typically breaks due to either abrasion or infection, so it's important to understand the underlying cause for better treatment.
However, Meikang ointment contains a mixture of ingredients with various effects and should not be used for skin tuberculosis, acute herpes simplex, smallpox, or chickenpox (as stated in the instructions).
If it is not one of these conditions, it should generally be fine to use.
If you are concerned, it is best to discontinue use.
For scrotal skin breakdown, it is essential to maintain cleanliness and dryness, avoiding contact with water.
If the issue occurs frequently, further investigation into the cause is necessary.
Wishing you good health.

More Info

Skin tears in sensitive areas such as the scrotum and foreskin can be quite distressing and may require careful management. These tears can occur due to various reasons, including friction from tight clothing, skin conditions, or even trauma. In your case, it seems that the initial tear was caused by tight clothing, which is a common issue.
When it comes to treating skin tears, especially in delicate areas, the choice of topical medication is crucial. You mentioned using a product called "皮新軟膏" (Pixin Ointment) initially, which is often used for skin repair and may help in promoting healing. However, the subsequent use of "美康藥膏" (Meikang Ointment) raises some concerns. While I cannot provide specific information about these products without knowing their exact formulations, it is essential to understand that not all ointments are suitable for open wounds or broken skin.

In general, when dealing with skin tears, the following treatment options are recommended:
1. Keep the Area Clean: Gently clean the affected area with mild soap and water. Avoid using harsh soaps or scrubbing, as this can further irritate the skin.

2. Moisturize and Protect: After cleaning, applying a suitable ointment that promotes healing without causing irritation is crucial. If the ointment you are using is not specifically designed for open wounds, it may be better to switch to a product that is. Look for ointments that contain ingredients like aloe vera, zinc oxide, or other soothing agents.

3. Avoid Irritation: It's essential to avoid tight clothing or any activity that may cause further friction in the area. Loose-fitting underwear made from breathable fabrics can help reduce irritation.

4. Monitor for Infection: Keep an eye on the area for any signs of infection, such as increased redness, swelling, warmth, or discharge. If you notice any of these symptoms, it is crucial to seek medical attention promptly.

5. Consult a Specialist: If your symptoms persist or worsen, it may be wise to consult a dermatologist or a urologist. They can provide a more thorough examination and recommend appropriate treatments tailored to your specific condition.

6. Consider Alternative Treatments: If the current treatment is not effective, discussing alternative options with your healthcare provider is essential. They may suggest different topical treatments or even oral medications if necessary.

In summary, while it is understandable to feel uncertain about the treatment you are receiving, it is always best to err on the side of caution. If you feel that your current treatment is not appropriate or effective, seeking a second opinion from another healthcare professional can provide you with peace of mind and potentially better treatment options. Remember, the goal is to promote healing while minimizing discomfort and preventing further injury.
