Recurrent tearing of the foreskin?
Hello, doctor.
I have been experiencing recurrent skin breakdown of the foreskin.
I have visited three hospitals, and they prescribed me different ointments: Xinghui Meikang cream, Ziyin ointment, and Dalai cream.
Previously, I used Meikang cream for balanitis, and it worked quite well.
However, this time, the foreskin has been repeatedly breaking after sexual intercourse (I have tested my blood sugar, and it is within normal limits).
After the wounds heal, there is a phenomenon of whitening and thickening, which is not painful, but when the foreskin is retracted behind the glans during erection, it feels tighter than before.
I would like to ask which ointment you would recommend I use first.
I have researched that for tight foreskin, ointments containing steroids are suitable, but prolonged use of steroids can also thin the skin.
I am a bit uncertain and would appreciate your guidance.

Hello: The types of ointments are generally similar; it depends on the care of the wound.
If not cared for properly, it may recur repeatedly.
Maintaining cleanliness and dryness is crucial, and it must be done.
Using antibiotics or steroids can cause the area to crack if pulled.
Wishing you good health.

Managing recurrent foreskin tears can be a challenging issue, particularly when it involves discomfort and potential complications from repeated injuries. It's essential to approach this problem with a comprehensive understanding of the underlying causes and the best treatment options available.
First, it's important to recognize that recurrent tearing of the foreskin can be attributed to several factors, including friction during sexual activity, inflammation, or skin conditions such as balanitis or lichen sclerosus. Given your history of balanitis and the recent episodes of tearing, it may be beneficial to address both the immediate symptoms and the underlying causes.
In terms of topical treatments, you mentioned three different ointments: 杏輝美康乳膏 (Miconazole), 紫茵素 (Zinc oxide), and 達來乳膏 (Hydrocortisone). Each of these has its own properties and indications:
1. Miconazole (美康乳膏): This is an antifungal cream that is effective against yeast infections, which can sometimes contribute to inflammation and irritation of the foreskin. If there is any suspicion of a fungal infection, this may be a good option to start with.
2. Zinc oxide (紫茵素): This is a barrier cream that can help protect the skin and promote healing. It is often used for diaper rash and can be beneficial in preventing further irritation from friction. If the skin is already damaged, applying a protective layer can help it heal without further injury.
3. Hydrocortisone (達來乳膏): This is a mild topical steroid that can reduce inflammation and itching. While it can be effective in treating inflammation, prolonged use can lead to skin thinning, especially in sensitive areas like the foreskin. Therefore, it should be used cautiously and typically for short durations.
Given your situation, where you have experienced tightness and discomfort after healing, it may be advisable to start with a non-steroidal option like the zinc oxide cream to protect the area and allow it to heal without further irritation. If inflammation or discomfort persists, you might consider using hydrocortisone for a short period, but it’s crucial to monitor for any signs of skin thinning or other side effects.
Additionally, it’s essential to consider lifestyle modifications to prevent recurrence. This includes:
- Gentle hygiene practices: Ensure that the area is kept clean and dry. Avoid harsh soaps or irritants that can exacerbate the condition.
- Lubrication during sexual activity: Using a water-based lubricant can reduce friction and help prevent tearing during intercourse.
- Avoiding tight clothing: This can help minimize friction and irritation in the genital area.
If these conservative measures and topical treatments do not lead to improvement, or if you experience recurrent infections or significant discomfort, it may be worth consulting a urologist or dermatologist. They can evaluate for any underlying conditions that may require more specialized treatment, such as lichen sclerosus or other dermatological issues.
In summary, while topical treatments can provide relief and promote healing, addressing the underlying causes and making lifestyle adjustments are equally important in managing recurrent foreskin tears. Always consult with a healthcare professional if you have concerns about your symptoms or treatment options.
