Choosing the Right Ointment for Recurrent Foreskin Irritation - Urology

Torn foreskin

Hello, doctor.
I have been experiencing recurrent skin breakdown of the foreskin.
I consulted two hospitals and had blood tests, which showed no issues with elevated blood sugar levels.
However, the two doctors prescribed different ointments for me: one is Gentian Violet and the other is Miconazole Cream.
I would like to ask which one I should apply first, as the ingredients and indications for both ointments are quite different.
Currently, I am applying Gentian Violet, and the redness and swelling of the foreskin have improved.
I would like to know what would happen if I switch to Miconazole Cream now.
(I also have a bit of eczema due to phimosis.) I want to understand which of these two ointments should be prioritized for recurrent skin breakdown of the foreskin.

Hello: First of all, both medications are acceptable; you can choose one.
Pay attention to keeping the foreskin clean and dry, avoiding moisture.
Wear loose-fitting underwear, and ensure safe sexual practices.
After treatment, if you do not apply the ointment for one month without recurrence, then it can be considered fully healed.
Wishing you good health.

When dealing with recurrent foreskin irritation, it's essential to choose the right ointment based on the specific symptoms and underlying conditions. In your case, you have been prescribed two different ointments: "人人紫茵素" (also known as "Ziyin Su") and "美康達來乳膏" (which is likely a topical corticosteroid). Each of these medications has different active ingredients and mechanisms of action, which can influence their effectiveness for your condition.

1. Understanding the Medications:
- 人人紫茵素 (Ziyin Su): This ointment typically contains natural ingredients that may have anti-inflammatory and soothing properties. It is often used for skin irritations, rashes, and minor wounds. If you have noticed improvement in the redness and swelling of your foreskin after using this ointment, it suggests that it may be effective for your specific irritation.

- 美康達來乳膏 (Meikang Dalai Cream): This cream likely contains a corticosteroid, which is effective in reducing inflammation and itching. Corticosteroids can be beneficial for conditions like eczema or dermatitis, which may be contributing to your symptoms.

2. Current Treatment:
Since you have already started using 紫茵素 and have seen some improvement, it may be wise to continue with it for a while longer. If the irritation persists or worsens, you could consider switching to the other ointment.
3. Switching Between Ointments:
If you decide to switch to 美康達來乳膏, it is generally safe to do so, but it’s important to monitor how your skin responds. Corticosteroids can sometimes cause thinning of the skin if used excessively, so they should be used judiciously. If you experience any adverse effects, such as increased irritation or a burning sensation, discontinue use and consult your healthcare provider.

4. Prioritizing Treatment:
In terms of which ointment to prioritize, if your primary concern is the redness and swelling, continuing with 紫茵素 may be beneficial, especially since you have seen improvement. However, if you notice that the irritation is not resolving or if you develop symptoms consistent with eczema or dermatitis, it may be appropriate to use 美康達來乳膏 for a short period to help control inflammation.

5. Additional Considerations:
- Hygiene: Ensure that you maintain good hygiene in the area to prevent further irritation. Gentle cleansing with mild soap and water can help.

- Moisturizing: If you have eczema-like symptoms, consider using a fragrance-free moisturizer to help keep the skin hydrated.

- Avoiding Irritants: Be mindful of any products that may irritate the skin, such as scented soaps or lotions.

6. Consulting a Specialist:
If your symptoms persist despite using these ointments, or if you experience recurrent episodes of irritation, it may be beneficial to consult a dermatologist or a urologist who specializes in male genital health. They can provide a more tailored treatment plan and may suggest additional therapies or diagnostic tests if necessary.

In summary, continue with the ointment that has shown improvement for you, and consider switching to the corticosteroid if needed, while being cautious about its use. Always keep an open line of communication with your healthcare provider regarding your symptoms and treatment progress.
