Menstrual delay

Hello doctor, my menstrual cycle has never been regular.
During my senior year of high school, it would sometimes be delayed by a month.
I thought it was due to late nights and the pressure of college entrance exams.
Now that I'm in my first year of college, my period has been delayed for two months and hasn't arrived yet.
I am getting enough sleep and don't have much stress.
What could be the cause of this? Is this considered normal?

Xiao Yu, 10~19 year old female. Ask Date: 2018/02/07

Dr. Huang Yiwen reply Obstetrics and Gynecology

Hello, Miss Hsiao-Yu: There are many causes of menstrual irregularities.
Some individuals may have thyroid dysfunction, elevated prolactin levels, or issues related to being overweight or underweight, as well as irregular lifestyle habits.
However, some people may undergo a series of tests and still not find a clear cause, which can be attributed to individual constitution.
It is advisable for you to visit a doctor for a thorough examination at the hospital.

Irregular menstrual cycles can be a source of concern for many women, especially when they experience significant changes in their cycle patterns. It’s important to understand that menstrual cycles can be influenced by a variety of factors, and what may be normal for one person might not be for another.
Firstly, let’s define what constitutes a normal menstrual cycle. A typical menstrual cycle ranges from 21 to 35 days, with the average being about 28 days. However, cycles can vary widely among individuals and even from month to month for the same individual. Irregular periods are generally defined as cycles that are shorter than 21 days or longer than 35 days, or if the timing of the cycle is inconsistent.

In your case, experiencing a delay of two months is certainly outside the typical range and warrants further investigation. There are several potential causes for irregular periods, including:
1. Hormonal Imbalances: The menstrual cycle is regulated by hormones, and any imbalance can lead to irregularities. Conditions such as polycystic ovary syndrome (PCOS) can cause hormonal fluctuations that result in missed or irregular periods.

2. Stress and Lifestyle Factors: While you mentioned that you are not experiencing stress currently, it’s worth noting that stress can have a lingering effect on your body. Additionally, lifestyle factors such as significant weight loss or gain, excessive exercise, or changes in diet can also impact your menstrual cycle.

3. Medical Conditions: Certain medical conditions, such as thyroid disorders, diabetes, or other endocrine disorders, can affect menstrual regularity. It’s important to rule out these conditions, especially if you have a family history of such issues.

4. Birth Control: If you are using hormonal birth control methods, they can also affect the regularity of your periods. Some forms of birth control can lead to lighter periods or even missed periods altogether.

5. Age and Development: Since you are in your first year of college, it’s also important to consider that your body is still adjusting to various changes, including those related to your reproductive system. Irregularities can be more common in younger women, particularly during the first few years after menarche (the first menstrual period).

Given that you have experienced a significant delay in your menstrual cycle, it is advisable to seek medical attention. A healthcare provider can perform a thorough evaluation, which may include a physical examination, blood tests to check hormone levels, and possibly imaging studies to assess your reproductive organs.
In summary, while irregular periods can sometimes be normal, a delay of two months is not typical and should be addressed. It’s essential to consult with a healthcare professional who can provide personalized advice and treatment options based on your specific situation. Remember, your menstrual health is an important aspect of your overall well-being, and seeking help is a proactive step towards maintaining it.
