Foreskin issues

Hello, doctor.
Recently, while masturbating, I accidentally applied too much force, causing my foreskin to retract to the coronal sulcus.
After the erection subsided, the foreskin did not return to its previous position.
When fully erect, I feel pulling and pain from the foreskin being stuck at the coronal sulcus.
During ejaculation, the glans also becomes engorged and painful.
I experience discomfort from the foreskin rubbing and getting stuck during masturbation, which severely affects my sex life.
I have tried to push the foreskin back, but to no avail.
I have not undergone circumcision, and my foreskin is somewhat long.
When not erect, it completely covers the glans, but when erect, about 90% of the glans is exposed.
Should I see a doctor for this condition, or can I attempt to push the foreskin back myself? Thank you for your time and response.

When it comes to issues related to the foreskin, particularly in the context you've described, it is essential to understand the anatomy and potential complications that can arise from improper handling or trauma to the area. The symptoms you are experiencing, such as pain during erection, difficulty retracting the foreskin, and discomfort during sexual activity, suggest that you may be dealing with a condition known as phimosis or possibly paraphimosis.

Phimosis is a condition where the foreskin cannot be fully retracted over the glans (the head of the penis). This can be a normal condition in young boys, but if it persists into adulthood, it can lead to discomfort and complications. In your case, since you mentioned that the foreskin is somewhat long and has been pulled back forcefully, it may have caused some inflammation or scarring, making it difficult to retract.

Paraphimosis, on the other hand, occurs when the foreskin is retracted behind the glans and cannot be returned to its normal position. This can lead to swelling and pain, and if not treated promptly, it can cause serious complications, including restricted blood flow to the glans, which can lead to tissue damage.

Given your symptoms, it is crucial to seek medical attention rather than attempting to resolve the issue on your own. Here are a few reasons why:
1. Risk of Complications: Attempting to forcibly retract the foreskin can exacerbate the problem, potentially leading to further swelling or injury. A healthcare professional can assess the situation and provide appropriate treatment without risking additional harm.

2. Proper Diagnosis: A medical professional can determine whether you are experiencing phimosis, paraphimosis, or another condition entirely. This diagnosis is essential for determining the best course of action.

3. Treatment Options: Depending on the diagnosis, treatment may vary. Options can include topical steroid creams to help loosen the foreskin, manual reduction techniques, or in some cases, surgical intervention such as circumcision or preputioplasty.

4. Pain Management: A healthcare provider can also help manage your pain and discomfort, ensuring that you can return to normal sexual activity without ongoing issues.

5. Education and Prevention: Consulting with a medical professional can provide you with valuable information on how to care for your foreskin and prevent similar issues in the future.

In conclusion, while it may be tempting to try to resolve this issue on your own, the best course of action is to consult a healthcare provider. They can provide a thorough examination, accurate diagnosis, and appropriate treatment plan tailored to your specific situation. Do not hesitate to seek help; your sexual health and overall well-being are important.
