Foreskin issues
Hello, I would like to inquire about the recent cold weather.
It's been so cold that my genitalia, specifically the corpora cavernosa, have shrunk, and my foreskin has retracted to the point where I can't even see the tip.
My scrotum has also swollen.
However, even with the recent warming, my genitalia have not returned to normal.
Now I am experiencing some discomfort and a dull pain, and my urination has become erratic.
What should I do?
Chun, 30~39 year old female. Ask Date: 2018/02/10

Hello: It is true that the genitals can shrink in cold weather, but it is not reasonable for the scrotum to swell.
Symptoms like discomfort, pain, and irregular urination are uncommon.
I recommend consulting a urologist at a hospital.
Wishing you good health.

Cold weather can have various effects on urological health, particularly concerning the penis and surrounding structures. The symptoms you describe—shrinkage of the penis, retraction of the foreskin, and swelling of the scrotum—can be attributed to several physiological responses to cold temperatures.
When exposed to cold, the body initiates a process known as vasoconstriction, where blood vessels narrow to conserve heat. This response is particularly pronounced in the extremities, including the genital area. The penis may appear smaller due to reduced blood flow and the contraction of the smooth muscle within the erectile tissue (corpora cavernosa). The retraction of the foreskin can occur as the body attempts to protect the sensitive tissues from the cold, leading to a temporary change in appearance.
The swelling of the scrotum may be due to a few factors. Cold temperatures can cause the cremaster muscle, which surrounds the testicles, to contract, pulling the testicles closer to the body for warmth. This contraction can sometimes lead to a feeling of tightness or discomfort. Additionally, if there is any underlying inflammation or irritation, cold weather may exacerbate these conditions, leading to swelling.
The discomfort and "dull pain" you are experiencing, especially if accompanied by changes in urination, could indicate a few potential issues. If the pain persists or worsens, or if you notice any unusual discharge, it is crucial to seek medical attention. Conditions such as urinary tract infections (UTIs), epididymitis (inflammation of the epididymis), or even testicular torsion (a medical emergency) could present with similar symptoms and require prompt evaluation.
In terms of management, here are some steps you can take:
1. Warmth: Keep the genital area warm. Wearing thermal underwear or using a heating pad can help alleviate discomfort and promote blood flow.
2. Hydration: Ensure you are well-hydrated, as dehydration can exacerbate urinary issues.
3. Gentle Massage: If comfortable, gently massaging the area may help relieve some tension and promote circulation.
4. Avoid Irritants: If you are using any topical products or soaps that may irritate the area, consider discontinuing them until the symptoms resolve.
5. Monitor Symptoms: Keep track of any changes in your symptoms, especially regarding urination. If you notice increased pain, difficulty urinating, or any other concerning signs, seek medical attention promptly.
6. Consult a Urologist: If symptoms persist beyond a few days or worsen, it is essential to consult a urologist. They can perform a thorough examination and may recommend tests such as ultrasound or urinalysis to rule out any underlying conditions.
In summary, while cold weather can lead to temporary changes in the appearance and function of the genitalia, persistent symptoms warrant a medical evaluation to ensure there are no serious underlying issues. Taking care of your urological health is essential, and seeking professional guidance is always a prudent step when experiencing discomfort or unusual symptoms.
